About the team.

The Colorado Refugee Wellness Center (CRWC) team is highly motivated to support refugees and immigrants in the greater Aurora area. The team provides individual and group mental health and psychosocial support services to a diverse array of populations and is looking for innovative ways to provide outreach to these communities.


About the Role.

This is a part-time role working approximately 20 hours per week. The Health Navigator is as an integral member of a supportive behavioral health team dedicated to providing culturally and linguistically responsive care to refugees and immigrants, consisting of behavioral health providers and health navigators. Assist refugee & immigrant community members with community outreach/education/advocacy, cultural/interpretation services, care coordination, case management, and resource provision. Serve as a cultural broker to enhance communication between staff, clients and refugee/immigrant communities. Offer cultural training/input to team. Possible participation on grants.


Responsibilities:

  • Operate as an integral part of a behavioral health team.
  • Provide language interpretation to clients receiving behavioral health services such as individual counseling, medication follow up, psychosocial skill building and wellness groups.
  • Provide first hand cultural knowledge as a member/leader in local refugee/immigrant community in Colorado. This includes helping to identify, understand and address the needs of refugee/immigrant communities.
  • Ability to build trusting relationships with community members and act as advocate for refugee community and liaison with diverse network of providers.
  • Provide outreach & health education to immigrant/refugee communities and share information about full range of programs, services, and how to access them.
  • Facilitate client and family engagement and optimize communication between clients and providers. Assisting with transportation, reminder calls, encouraging regular attendance at appointments, care coordination, and case management.
  • Receive ongoing training and team support from supervisors, therapists, peers and medical providers. Complete patient navigation and interpretation training, as needed.
  • Help conduct initial screenings/interviews for immigrants/refugees identified as at-risk for behavioral health issues.
  • Help translate health and mental health education material into primary languages as needed.
  • Connect clients and their families to various community resources, referrals, advocacy, service planning. This may involve telephone contact, home visits, and meetings.
  • Provide cultural education/training for providers.
  • Assist with other tasks and projects as needed, including grants.
  • Staff member will monitor and coordinate care for assigned clients in partnership with the other members of the clients care team.

About you.

The ideal candidate would have a strong desire to assist refugee/immigrant communities and have familiarity with them and their related needs and services. S/he would work well collaborating with a multidisciplinary team and outside partners, have excellent bilingual language skills and be flexible and patient in the face of changes or challenges. Candidates should be very reliable, have strong independent work skills, and be willing to share cultural knowledge with our team.


  • NOTE: This person MUST be bilingual in the languages listed below.


Requirements:

  • High school graduate or GED required. Bachelor's degree preferred in a human services field, foreign or domestic, or in a non-human services field with one year relevant job experience.
  • One year of direct service provision in human services/ navigation or a related field preferred.
  • Primary Language Requirement: Bilingual English/Tigrinya
  • Additional Language needed: Amharic
  • Strong desire to serve refugee/immigrant communities.
  • Excellent listening, oral, and written communication skills.
  • Familiarity with needs of refugee/immigrant communities.
  • Flexibility, ability to work independently
  • Ability to work well collaborating with others on diverse interdisciplinary behavioral health team.
  • Ability to complete health navigation training, as needed.
  • Demonstrated cultural and linguistic competency.
  • Ability to serve as cultural broker and interpreter.
  • First hand cultural knowledge as a member/leader in local refugee community in Colorado.
  • Ability to build trusting relationships with community members and act as advocate for and liaison between refugee community and diverse network of providers.
  • Skilled at facilitating client and family engagement.
  • Ability to give and take direction accurately, efficiently and flexibility.
  • Basic computer competency


Starting Salary for this role.
(based on experience and company equity):

  • $17.65- $26.48 hourly


Language Differential Pay.
(For Bilingual Candidates):

  • 10% of base salary up to $6,000/year.
