Under the general direction of the Manager of Healthcare Relations, the Healthcare Representative will educate medical referral sources about Community Hospice of Northeast Florida’s services, access to hospice care and delivery of end-of-life care with a strong emphasis on customer service.

They will be well versed in appropriateness of services and will be able to educate on a one-to-one basis as well as large group settings. Facilitates speed to care by providing a range of preadmission and admission services. Collaborates with internal and community partners. Provides education regarding Community Hospice services and philosophy. Collaborates with the manager, physicians, nurses and case managers to ensure the best outcome for patient, families and referral sources.

Contributes to the smooth function of services, advocating for patient and family needs, coordinates care and facilitates transfer of care to appropriate team. Understands Medicare Conditions of Participation for Hospice, how insurances pay for Hospice and State Regulations that apply to Hospice admission. Provides education and information to referral sources, physicians, patients, and their families. Represents the organization in a positive professional manner understanding the importance of the first impression.

  • Cultivates new and maintains existing relationships with physicians, hospitals, nursing homes and assisted living facilities.
  • Educates medical community about the benefits of hospice care, what hospice appropriate referrals look like, and patient benefits of early referrals
  • Achieves quarterly and annual referral and admission goals within assigned territory as set by the Manager of Health Care Relations.
  • Develops territory management plan for complete coverage of assigned territory, including six to ten daily physician calls per day and various visits with office and hospital staff.
  • Identifies doctors in territory who have patients with hospice referral potential and develops plans to increase hospice referrals.
  • Leverages formal and informal in-service education events to insure delivery of Community Hospice messages.
  • Maintain accurate record of all expenses and report to Community Hospice on a timely basis, ensuring up-to-date business development records within assigned database.
  • Validate all reported referral and admissions are reflected properly within Community Hospice records weekly and finalized within 30 days of the end of the month.
  • Collaborate with other members of the hospice team within your assigned territory.
  • Maintain memberships in related health care professional organizations for the purpose of networking.
  • Communicate issues that arise in the field with clinical staff through defined channels to enhance service delivery.
  • Participate in organizational committees as identified that enhance the working relationships between departments within Community Hospice.
  • Promote Community Hospice through participation in community and professional organizations.
  • Ability to implement Medicare Local Coverage Determination to assess patients for Hospice eligibility. Collects and communicates appropriate information to/from admission physician to determine hospice eligibility.
  • Maintains the confidentiality of patients and families in compliance with the Health Insurance Portability and Accountability Act (HIPAA).
  • Provides patients and/or family, Admissions Staff, and Community Partners consultation and information regarding Health Care Surrogate, Health Care Proxy and other Advance Directives.
  • Maintain the confidentiality of patients and families within Community Hospice as per government mandates.

Required Licenses/Certifications:

  • Current State of Florida driver’s license with driving record acceptable to Community Hospice insurance carrier, and reliable transportation.

Education/Experience:

Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:

  • Bachelor’s degree or equivalent required.
  • Minimum two years of documented sales success, preferably in selling services to medical professionals within the service area.
  • Fluent English, secondary language a benefit.

Knowledge of:

  • Ability to communicate and articulate Community Hospice services and benefits to medical professionals in a clear and concise manner.
  • Excellent presentation skills for both small and large audiences with strong communication skills
  • Ability to work autonomously and support fellow team members.
  • Customer relationship management and sales force automation systems.

Pay: $55,000.00 - $65,000.00 per year
