Immeidate staffing needed at these locations; BEACHWOOD 44122 (28hrs/wk), MENTOR 44060 (30hrs/wk), EUCLID 44117 (44hrs/wk), CLEVELAND 44118 (45hrs/wk).

First Light Home Health Care is looking for Home Health Aide who can provide home care services to our clients and follow as assessed in care plan and in compliance with client contract agreement

The Home Health Aide is responsible for the provision of a number of services to meet the client’s personal care needs and to promote their comfort and independence.
Scope of Duties and Responsibilities:

• Assist with personal care activities, including:
• Assist with household tasks directly essential to the client’s health:
• Meets safety needs of client, including safe use of equipment (foot stools, side rails, step stools, wheelchairs, etc.)
• Immediately reports any changes in client’s conditions or incidents to Affordable Home Care.
• Accurately documents services provided in accordance with the agency policies and procedures on the day services are rendered. Submits documentation in a timely manner.
• Attends staff development programs as appropriate for Home Health Aides.

Knowledge, Skills and Abilities Required:

• Completion of authorized training program or documentation of one (1) year employment as an aide in an institution or home care setting is desired.
• Meets hiring criteria as defined in the agency’s minimum standards.
• Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agency’s client and their family.
• Must accept responsibility for maintaining skills, learning and adhering to agency’s policies and procedures.

Major Responsibilities:

• Responsible for assisting the agency’s clients with their personal care and hygiene needs in accordance with the personal services care plan.
• Responsible for assisting the client in performing activities of daily living in a safe and beneficial manner.

Physical Qualifications:

• Able to work an average of 20 to 40 hours per week.
• Able to lift up to 50 pounds.
• Able to work in a stressful environment.
• Able to verbally communicate effectively.
