We are currently hiring for a High Acuity Care Manager in our Buffalo Area! As a High Acuity Care Manager at Venture Forthe, you will be responsible for the coordination of all medical and psychosocial care for the Health Home Plus consumers. The Care Manager establishes relationships with the client, his/her supports and all service providers in order to work towards ensuring the client’s needs are being met.

CORE RESPONSIBILITIES:

  • Maintain a caseload of approximately 20 high acuity clients with a minimum of four monthly contact with each client, in addition to collaboration family members and service providers.
  • Complete Comprehensive Assessments to identify problem areas, and needs for the client
  • Assess client’s immediate needs and develop and implement action plan if necessary
  • Assist with development of Person-Centered Plan of Care for each individual client
  • Provide comprehensive care coordination and treatment planning
  • Participate in transitional care from inpatient settings to community, including post discharge follow up
  • Coordinate with service providers and informal supports involved in care, advocating for the client and his/her needs
  • Knowledge of community resources
  • Maintain documentation of outreach, engagement and enrollment process for each client, including but not limited to progress notes, assessments, plans and contacts with all service providers
  • Complete any/all paperwork and documentation by set deadlines
  • Complete chart audit corrections on a monthly basis
  • Follow up with client needs in a timely manner, providing Face-To-Face contact in the client’s home and/or a community setting at least twice per month
  • May have 24/7 on call responsibilities

EDUCATION AND EXPERIENCE

1. A Master’s degree in one of the qualifying fields4 and one (1) year of Experience; OR

2. A Bachelor’s degree in one of the qualifying fields and two (2) years of Experience; OR

3. A Credentialed Alcoholism and Substance Abuse Counselor (CASAC) and two (2) years of Experience; OR

4. A Bachelor’s degree or higher in ANY field with either: three (3) years of Experience, or two (2) years of experience as a Health Home care manager serving the SMI or SED population.

SPECIALIZED SKILLS:

1. Providing direct services to people with Serious Mental Illness, developmental disabilities, alcoholism or substance abuse, and/or children with SED; OR

2. Linking individuals with Serious Mental Illness, children with SED, developmental disabilities, and/or alcoholism or substance abuse to a broad range of services essential to successful living in a community setting (e.g. medical, psychiatric, social, educational, legal, housing and financial services).

  • Ability to manage time management efficiently
  • Excellent written and verbal communication skills
  • Demonstrate ability to interact with community providers and colleagues professionally
  • Proficiency with Microsoft Office and GMAIL
  • Ability to meet and maintain deadlines

MISCELLANEOUS:

  • Must possess clean NYS driver’s license and reliable transportation
