PRIMARY PURPOSE
Responsible for all aspects of patient medical records to include electronic medical record applications, filing, retrieving, tracking records, preparing and scanning documents, etc. to ensure that medical records are available, accurate and current.

MINIMUM SPECIFICATIONS Education
  • Must have High School diploma or GED.

Experience
  • Prefer office experience with use of software applications in work processes, preferably in a health care environment.
  • Prefer one (1) year of HIM experience.

Certification/Registration/Licensure
  • None

Skills or Special Abilities
  • Must successfully complete required training and meet prescribed performance standards within 90 days of employment.
  • Must be able to operate multi-line phones, faxes, and copy machines.
  • Must be able to comprehend and apply established policy and procedures.
  • Must be able to demonstrate good decision-making skills.
  • Must be detail oriented and exhibit effective organizational skills.
  • Must have good written and oral communication skills with patients, visitors, and hospital staff.
  • Must have a clean driving record.
  • Prefer knowledge of Electronic Medical Record (EMR) software applications.
  • Prefer knowledge of Health Information Management or Healthcare Operations Applications procedures.
  • Prefer knowledge of document imaging preparation and scanning.

Responsibilities
1. Utilizes electronic computerized master patient index, chart tracking, and Electronic Medical Record applications, to create the record, update, verify and review patient medical records numbers and record volume, record location, record history and activity, and record completion.
2. Responsible for performing workflow processes and procedures associated with data entry and documentation into electronic applications and the Electronic Medical Record.
3. Reviews documentation and data in electronic applications, the Electronic Medical Record and the paper medical record in order to process requests for medical records.
4. Receives requests for medical records and documentation and provides routine information such as medical record numbers, admission or discharge dates and chart locations to authorized requestors and retrieves medical records for authorized users.
5. Prepares and scans documents into the document imaging system.
6. Reviews and facilitates medical record and data requests, retrieves medical records from files and assures that medical records are tracked and requests followed-up and that issues are resolved.
7. Reviews paper documentation for patient name and medical record number to ensure proper patient identity is confirmed. Files, assembles, and merges documents and loose reports into the medical record folder.
8. Files medical record folders into the appropriate file locations in terminal digit order per procedure.
9. Meets productivity, timeliness, and quality standards and goals set by the HIM Division. Provides data for monitoring productivity and outcomes through maintenance of spreadsheets, databases and other documentation procedures.
10. Occasional driving van to pick up records from various locations.
