Home Instead is looking for a Assessment Nurse to support our growing home care business. Flexible for Part Time or Full Time. The Assessment Nurse will be responsible for initial client assessment/sign up, 60-day reassessments, client care plans, caregiver supervision and introductions, and caregiver training.

Responsibilities include:

• Performing in-home client assessments and creating individualized care plans based on clients' specific care needs
• Signing up new clients
• Conduct care consultations with clients and families to discuss their needs and provide solutions
• Conducting periodic telephone/virtual care assessments
• Revising care plans as needed
• Supervising home care aides (CHHAs)
• Teaching and training the clinical portion of CAREGiver orientation/skills assessments

Qualifications include:

• Current Registered Nurse license in New Jersey
• 2 years’ experience in home care, home health care, hospice, or community health organization
• Prior RN experience in home care or home health care preferred
• Must be self-motivated, organized, and willing to work with little direction
• Ability to work well in a collaborative team environment
• Has a valid driver’s license with a reliable vehicle, and willing to travel to the clients’ homes
