Position: Home Health & Hospice Nurse

Description: The Home Health & Hospice Nurse works as a primary case manager for home health and hospice patients and families in their homes, nursing homes or inpatient facilities. He/she is responsible for providing assessment, interventions, evaluation of patient/family needs, care coordination and case management for his/her caseload of patients. Services are provided to those of all ages; infants to elderly.

Number of Hours: Full-time: 32-40 hrs./week

Schedule: 8:00am – 4:30pm weekdays, generally 4 days/week. On - Call every 8th weekend and holiday. 2 - 4 weeknights of call per month.

Educational Requirements: RN licensure

Other: Home Health or Hospice experience preferred, two or more years of nursing experience is also preferred. Travel required within 30 mile radius of Orange City. Mileage for work-related travel reimbursed

Pay: $28.00 - $34.90 per hour
