HIRING HOMELESS OUTREACH NAVIGATOR

Position: Homeless Outreach Navigator

Salary: $22.00 to $26.00 per hour (Depending on education, experience, skillset and hiring site)

Summary: Under the direct supervision of the Program Manager, the Homeless Outreach Navigator will focus on recruiting and build relationships with the homeless population, provide advocacy, and assist with connection to benefits and housing.

Essential Job Functions:

  • Collaborate and develops on-going working relationships with local law enforcement and courts.
  • Engage in outreach activities to recruit participants for the Homeless Assistance Program.
  • Respond to community requests for outreach interventions.
  • Work collaboratively with the Homeless Outreach Coordinator to conduct outreach in the community, focusing on those who are vulnerable.
  • Provide advocacy for homeless neighbors until shelter or permanent housing is obtained.
  • Facilitate group level interventions.
  • Collect community assessment data and analyze information collected.
  • Assist in obtaining housing readiness documentation
  • Complete risk assessments and other documentation for data reporting.
  • Work with Navigators from other agencies in the region to provide team outreach services.
  • Assist in identifying appropriate housing and completing housing applications.
  • Provide support for people as they transition from homelessness to housing through intensive case management.
  • Maintain complete client records (electronic data base- HMIS), daily activity logs, mileage logs, and other reports as directed.
  • Establish and maintain positive, productive working relationships with mental health programs, shelter programs, police (and other local officials), and providers of services and resources to homeless neighbors.
  • Attend team meetings, case conferences, training workshops and community meetings as needed.
  • Maintain HIPAA compliance per State and Federal guidelines.
  • Other duties as assigned by the Program Manager and Executive Director.

Qualifications:

  • 1 year experience supporting and connecting clients to housing programs.
  • Has basic understanding of severe mental health, physical health, and substance abuse issues and symptoms.
  • Well versed in public social services to link patients to care/resources.
  • Knowledge of communicable disease prevention, care, and resources along with co-factors related to health, mental health, substance abuse, socioeconomic status, stigma, etc.
  • Good verbal, written and telephone skills.
  • Displays appropriate, professional, healthy boundaries and exercises mature judgment and understanding of safety concerns
  • Competence in software programs including Word, Excel, PowerPoint, Access, and publishing program required.
  • Understanding of homeless population, Substance Use Disorder treatment & prevention and related psychosocial issues.
  • Speaks conversational Spanish (preferred)

  • Must have a valid CA Driver License and must be able to provide a clear recent Motor Vehicle Report.
  • Knowledge of medical care treatment options and emotional support programs.

Education:

  • BA degree social work or related field or 2-years of equivalent social services experience preferred.
  • High School Diploma required.
  • Registered as AOD Counselor; certified is preferred.

Working Conditions:

  • Work may be outdoors and in an office.
  • Transporting from business to business.
  • Requires ability to work as a part of a team.
  • The ability to work in a stress free environment.
  • Work hours vary and require some weekends and holidays.
  • Driving is required.

Must be able to pass the following pre-employment screenings:

  • Background check, including criminal history (LA CADA considers all applicants equally and will take into account each applicant’s individual life circumstances).
  • Drug and health screen, including negative TB test
  • Employment and education verification

Additionally, L.A. CADA offers HIV / AIDS prevention and intervention services, including HIV testing.
