What You Will Do:
Assist and counsel hospice patients and families with health related financial, social and emotional concerns

according to the physician’s orders.

Requirements:

• Has a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education and
• Has 1 year of social work experience in a healthcare setting.

You will visit patients and families in their homes which could be a single family dwelling, apartment or condo, assisted living facility or skilled nursing facility. This position allows you to spend quality one on one time with your patients at a time when it is most needed
