Embracing HospiceCare is seeking highly motivated and energetic Registered Nurse Case Managers with at least 1year of hospice or home health experience to cover Ocean and Mercer County.
The Hospice RN Case Manager establishes a professional and caring relationship with our patients and families, support staff, physicians,discharge planners, social workers, facilities and other health careprofessionals by providing hands-on care, on-going clinical management and educationregarding hospice care.
Job Requirements:
- Registered Nurse with a current NJ RN license
- Covid Vaccination + Booster
- Current vehicle liability insurance and license
- Excellent people, verbal and written communication skills
- Ability to adjust to daily needs of patients
