Responsibilities: Job Summary
The Hospitalist Team Coordinator provides highly knowledgeable and skilled administrative support to hospitalist physicians and various hospitalist team staff. Acts as a liaison with the Hospitalist Medical Director on specific projects designed to improve the hospitalist program which may come from the Hospitalist Medical Director, other hospitalists, administration, or other hospital staff. Required to manage their own duties and functions independently, maintaining a high quality for detail, timeliness and must maintain the integrity of highly confidential information. This position requires critical thinking skills and the constant exercise of a high degree of independent judgment in response to complex sensitive issues, interpersonal communication skills, decision-making, discretion and troubleshooting. Must accomplish assignments in an accurate and timely manner to ensure efficient operations of the administrative functions.
  • Communicates with PCPs about admissions, ensuring appropriate inpatient hospital stay data/reports have been sent to PCPs and subspecialists’ offices. Serves as a liaison for other departments, physicians, family members of inpatients, pharmacies, pharmaceutical representatives, insurance companies, etc.
  • Maintains the Hospitalist Medical Director’s schedule. Establishes, coordinates and communicates meetings and huddles as necessary. Coordinates the onboarding for various staff in collaboration with the Hospitalist Medical Director. Advocates the Patient Experience initiative for the hospitalist program and hospital. Receives and monitors calendar template of requests for vacation/time off. Monitors compliance with hospital committee attendance.
  • Performs follow-up on post discharge communication. Distributes discharge summaries expeditiously to providers that do not have access to Electric Medical Record. Manages calls from patients, family members and providers to assist with transitional care. May serve as a patient advocate by conducting discharge follow-up calls and forwards questions to appropriate Hospitalist. May assist patients with scheduling appropriate follow-up appointments with PCP or Specialist. Obtains medication clarifications and forwards prior authorizations.
  • Compiles and analyzes data on daily, weekly, monthly, and as needed basis related to patient satisfaction, process improvement, program data tracking and other requested reporting data. Provides statistical reports that monitor and identify trends; analyzes data to ensure data integrity.
  • May perform other duties as assigned.
