The HHC (Household Coordinator) provides leadership to the household in activities, assuring each household’s abilities to promote the psychosocial well being of all residents and to plan and carry out interests and social activities for residents and the household, including activities creating a diversity of opportunities for meaningful pursuits, daily pleasures, recreation, exercise, friendships, social interaction, growth and learning, fun and enjoyment, personal routines and household rituals. The HHC works to grow the household staff in activities expertise. Ensures that residents complete all necessary documents such as power of attorney and living will in order to direct decisions that may be made on their behalf. Determines what kinds of support services residents may need to promote their well-being. Facilitates the effective completion of care plans and RAI documentation with regards to psychosocial well-being, social service and activity needs. The HHC works cooperatively and respectfully with residents, coworkers, families, visitors and volunteers to create a positive and pleasant environment in the home. This may involve direct problem solving, conflict resolution and ensuring that the psychosocial and activity needs and desires of residents are met.
DUTIES AND RESPONSIBILITIES OF THIS POSITION
  • Providing knowledge and skills to staff in their household, assuring that the household has the needed resources (community links, materials and supplies, skills and knowledge). – Assuring the hiring, orientation, training and evaluation of all staff in his/her household.
  • Assuring regulatory compliance by encouraging appropriate record keeping and charting in their household and by maintaining appropriate files to document compliance.
  • Ensuring that their household participates in Quality Improvement measurements that will surface questions and issues to address and that appropriate follow up is taken to assure quality outcomes, performing professional audits and observations to confirm the competency of household staff.
  • Ensures that activities and social service functions in the household are in compliance with Federal and State regulation and professional standards of practice while honoring the values of person-centered, resident-directed practices.
  • Provides leadership to household staff, and supervision to Homemakers, for quality of life enhancement practices and services that support the values of person-centered, elder-directed practice.
  • Facilitates each household’s abilities to plan and meet the psychosocial and activity needs of the residents and the household, including creating diverse opportunities for meaningful pursuits, daily pleasures, recreation, exercise, friendships, social interaction, growth and learning, fun and enjoyment, personal routines and household rituals.
  • Assures that resident rights, preferences and responsibilities are honored and met, including the right to refuse or individualize treatment, and monitors the community and household response to resident concerns through continuous quality improvement.
