Job description
The Housing Case Manager offers high-quality case management interventions to clients enrolled in the Encompass Links2Health Program such as review of housing status, housing needs, social determinants of health impacting housing sustainability, and goal-setting. In addition, the Housing Coordinator partners with clients to offer coordination with landlords and other community providers. The following Evidence-Based Interventions are used: Motivational Interviewing and; Strengths-Based Model. The Housing Case Manager, along with the Case Management team will collaborate to achieve care plan goals. The Housing Case Manager is expected to demonstrate exceptional flexibility, willingness to learn, ability to work effectively on a team, empathic and strengths-based skills, and time management skills.
Responsibilities:
- Facilitate and review assessment documents and identify appropriate housing options
- Facilitates goal-setting and follow-through using Motivational Interviewing to partner with patients on improving follow-through with health and wellbeing goals
- Effectively manage crisis by using Motivational Interviewing and De-escalation skills, both in support of clients and other team members
- Effectively communicate care plans with team members at Encompass & other providers in the community
- Offer exceptional care coordination skills that include warm hand-offs and closed-loop referrals
- Effectively track progress using efficient and accurate documentation
- Understanding of transdisciplinary teams and roles using an integrated approach to care
Qualifications:
- 2+ years of experience with any of the following:
- working with individuals experiencing homelessness;
- providing housing navigation;
- case management services;
- experience supporting individuals with co-occurring disorders
- Strong computer skills, including able to easily navigate around health care systems
- Must have valid CA driver’s license and clean driving record
- Must be fully vaccinated for COVID19
- Preferred: Bilingual/bicultural
Wages & Benefits:
$3,500 appreciation bonus at successful completion of 120 day probation period. Our total compensation package includes a comprehensive medical and dental insurance plan for full time employees, 403(b) retirement plan, paid holiday, sick time, and 17 days of paid vacation, pro-rated for positions less than 40 hours a week.
