Catholic Guardian Services is licensed by the New York State Department of Health as an Article 29-I Voluntary Foster Care Agency (VFCA) Health Facility. This licensure allows us to provide limited health-related services to our foster children and enables us to contract and bill Medicaid Managed Care Plans.
The I/DD Coordinator is responsible for working with parents, foster parents, and the case planning team to help get children and youth in care certified for OPWDD services, most of which will be delivered post-discharge; develop a plan for post-discharge services and housing options; coordinate with the ABA in-house lead all to ensure that children and you in care with an I/DD diagnosis are supported and receive appropriate services.

Through the delivery of Core Limited Health-Related services and Other Limited Health Related Services we are able to maintain the physical, mental and psychosocial well-being of each child in our care, as determined by his/her assessments and treatment plans.
Job Summary:
To assist individuals with developmental disabilities to obtain supports and services offered through the Home and Community Based Medicaid Waiver, determined through a person-centered approach in cooperation with the individual’s circle of support, including family, advocates, Care Managers, case planning team, and service provider(s).
Major Duties
- Coordinates contact, communication and meetings with individual and circle of support members.
- Reviews individuals service plans, evaluations, case records, etc. to learn about individuals’ valued outcomes, goals, and dreams.
- Conducts person-centered planning interview(s) with individual and members of their circle of support to determine new service recommendations based on expressed plans, goals and dreams
- Based on information obtained through the person-centered interviews, and in conjunction with the case planning team, family, and Care Manager, develops the Individual transition plan for each individual; shares the transition plan with the individual and their circle of support.
- Monitors implementation of the transition plan.
- Assists the individual, the case planning team, and the family in having the child deemed eligible for OPWDD services where necessary.
- Assist the case planning team and Care Manager in implementing each individual’s transition plan through the identification of service providers.
- Functions as an advocate on behalf of the individual and their family to ensure delivery of supports and services.
- Responsible to maintain accurate and complete documentation for all assigned individuals receiving transition coordination services and for tracking progress in meeting Balanced Incentive Program goals.
- Coordinates with the Administration for Children’s Services office for children with I/DD.
- Any other responsibility as deemed necessary by the A29I Director.

Pay: $55,000.00 - $65,000.00 per year
