SUMMARY

This position will work remotely most of the time. ElderSource has a centralized intake system, under the Aging and Disability Resource Center (ADRC), to provide access to long-term care services. Intake Specialists administer standard intake and screening instruments for the purpose of gathering information about an applicant for services. Staff also follow-up with clients waiting for services to review any changes in their situations. Screening ensures prioritization for funded programs including General Revenue Programs and the Statewide Medicaid Managed Long-term Care Program (SMMCLTCP).

I. Essential Duties and Responsibilities:

  • Accept referrals of clients over the age of 60 from the ADRC Helpline as well as other organizations for intake and screening utilizing a standardized screening instrument.
  • Determine the individual?s needs using the standardized screening for potential eligibility for programs including Medicaid-related services in an efficient, timely and consumer friendly manner.
  • Contact individuals on the Assessed Priority Consumer List (APCL) as required to update information and complete an annual re-screening within the Department of Elder Affairs (DOEA) mandated time frame.
  • Explain Medicaid eligibility rules and the Medicaid eligibility process to prospective applicants. (LTC Education)
  • Input client screening into CIRTS (Client Information and Registration and Tracking System).
  • Post information in the enrollment screen of CIRTS related to APCL as needed.
  • Provide necessary forms required, including HIPAA and Social Security notice.
  • Provide the Medical Certification form (5000-3008) to clients on the wait list for Statewide Medicaid Managed Care Long-term Care Program (SMMCLTCP) and who meet the criteria defined by DOEA at the time of screening or re-screening.
  • Provide screenings and re-screenings at the client's home if required.
  • Receive referrals from Adult Protective Service for medium and low risk referrals.
  • Receive Aging Out referrals from the Department of Children and Families (DCF) and process them for enrollment in DOEA funded programs.
  • Assist the Intake and Screening Supervisor with the release of clients to the Lead Agency for Community Care for the Elderly (CCE), Home Care for the Elderly (HCE), and Alzheimer?s Disease Initiative (ADI) programs when funding is available.
  • Generate units of Intake and Screening service.
  • Meet productivity standards as required to achieve budget and address workload.
  • Work with Helpline staff to ensure appropriate triage of referrals to Intake.

Intake staff must have the following knowledge and skill set in order to perform Intake functions:

  • Knowledge of computer applications to perform the functions of the position, including word processing, database, and spreadsheet applications;
  • The ability to work independently and with minimal supervision;
  • Knowledge of available ADRC administered programs and available Medicaid programs;
  • The ability to work with disabled adults, elders, caregivers, stakeholders, and community partners in a knowledgeable, engaged, and compassionate manner; and
  • The ability to set and track personal performance goals to efficiently manage workload.

III. Certificates, Licenses, Registrations

  • Must be certified by completing the Aging and Disability Resource Centers Long-Term Care Program Education Certification Survey.
  • Must be certified by the Department as documented by the completion of the Department approved DOEA forms 701S and 701B training modules.
  • Signed acknowledgement that the employee has received, read, and understands the ?Prohibited Activities?.

IV. Education and/or Experience

  • Have a Bachelor?s Degree from an accredited college or university; or
  • Have an Associate Degree from an accredited college or university and a minimum of one (1) year of experience as a caseworker, case manager, intake specialist, or experience in performing human services related work; or
  • Have a high school diploma or GED and two (2) years of experience as a caseworker, case manager, intake specialist, or experience in performing human services related work.

Pre-employment background check and drug screen will be conducted on top candidate. It is mandatory that a candidate be able to pass a Level II criminal background check through the Department of Elder Affairs prior to starting work.

Salary: $35,000.00 / year
