Job description
Provides targeted case management services to ensure mental health consumers who have a Serious Mental Illness and are Literally Homeless have access to all services that are necessary for tenure and progress in the community.Case Manager has a small caseload and will transition consumers from shelter to permanent housing by using Critical Time Intervention Techniques.
Performs duties in support of the Medical Center mission to ensure the highest quality of client care in an economically sound and efficient manner.
This position serves: Adult consumers 18 years of age and up.

Schedule: FR =Full-time Regular, 1 =Day Shift, 8:30-5:00
