- Demonstrate commitment to the social emotional, behavioral, and academic success of all students
- Co-facilitate a district level multi departmental team to align community-based resources for the most at-risk students (District Student Intervention and Support team)
- Collect and document comprehensive information from schools and family
- Partner with community agencies and mobilize community resources
- Collaborate with school leaders and school-based intervention teams to discuss student needs, develop intervention plans, assist with community agency referrals, monitor student progress
- Recognize the relationships and comprehensive systems of care among schools and community agencies (e.g. FAPT, CSB, DSS, J&DR Court Unit)
- Initiate referrals to the FAPT team to seek student/family resources using Children Services Act funds
- Audit TSIS process in schools and provide school leaders with feedback
- Assist the Director of Student Services with the discipline process and attendance monitoring
- Monitor impact of services recommended and required by the DSIS
- Demonstrate understanding of Special Education procedures and services
- Provide professional development relative to initiative of the student services department
- Develop effective working relationship with schools, families, and community agencies
- Communicate effectively with internal and external stakeholders
- Utilize effective documentation and organizational skills
- Maintain strict confidentiality regarding students and families
- Demonstrate computer competency and familiarity with a variety of technologies
- Work innovatively to help achieve the vision and mission of Hampton City Schools
- Candidates must possess a Bachelor's Degree in education or related field (Education, Educational Leadership, Counseling, Social Work, Psychology)
- Experience working with families of/and children in crisis required.
- Experience in K-12 instruction and/or instructional leadership experience preferred.
- Aptitude with student intervention systems, Google suite, and web based data platforms preferred.

DUTIES AND RESPONSIBILITIES:
Under the general supervision of the Director of Student Services, this employee is responsible for planning, implementing and evaluating the TSIS (Targeted Student Intervention Support) teams and the DSIS (Division Student Intervention Support) team. This employee provides assistance and support for school teams as the school staff design and implement academic and behavioral interventions. This employee collaborates with various HCS departments, city and community agencies and families to ensure the needs of students who are at-risk for academic, behavioral and/or attendance are met.
