Job description
Under general supervision, employee is responsible for providing case management services and aggressive outreach to mental health and substance abuse clients that are referred by the Sheriffs departments in the catchment of the Middle Peninsula Regional Security Center. Referrals will also be made by CSB staff; jail staff and/or probation officers. The jail diversion counselor will participate in community advocacy groups i.e. SAARA and NAMI. Case management services include monitoring court dates, hospital discharge planning; assisting clients in identifying employment opportunities, living alternatives, educational, and entitlement opportunities. Employee provides crisis diversion activities on an on-call basis as assigned by the Director of Clinical Services. Employee will intervene with the Sequential Intercept Model and aggressive outreach. Employee must exercise tact and courtesy in contact with staff, clients, families, public, and service providers.

BA/BS in Human Services, Social Work, Psychology, Sociology or related field, and I to 2 years experience desired in emergency/crisis services, case management, or law enforcement; or any equivalent combination of training and experience which provides the required knowledge, stuffs, and abilities.

Must possess a valid Virginia Driver's License. Must obtain a TB test with satisfactory results within 30 days of employment and annually thereafter as required by licensing authority.
