For nearly 70 years, Stella Maris has been a leading nursing home, providing Baltimore-area residents with a comprehensive range of health and residential services.

We are currently recruiting for a licensed Social Worker to join our team. This is an exciting opportunity to join our mission driven organization to provide excellent care to our residents. This role will support our long-term care and rehab units.

$2,500 Sign On Bonus for Full Time

JOB SUMMARY:

This position provides direct social work services to nursing home and rehabilitation patients and families.

JOB RESPONSIBILITIES:

· Services include admission, psychosocial assessment, plan of care, linkage with community resources, counseling, crisis intervention, and discharge planning.

· Responsible for required documentation in compliance with State and federal regulations.

· Facilitates effective communication between patient/family and clinical team members.

· Provides appropriate information and referral services to patients and families/caregivers.

· Assumes responsibility for discharge planning

QUALIFICATIONS:

· BSW or MSW in Social Work

· Licensed Social Worker in Maryland (LM, LCSW, LCSW-C)

· Experience in hospice or medical setting

· Strong discharge planning experience

Pay: $50,000.00 - $80,000.00 per year
