In general, the work of the LPN is a professional, medical position supporting individuals with disabilities receiving services through Community Interactions, Inc. The responsibilities of this position include assisting the individuals in achieving their desired outcomes as identified in their Annual Plan, performing general office duties, requiring the ability to sit and/or stand for long periods of time. Work outside of the office setting is also expected and visits to homes and other work sites. Visits to areas outside the office which could include lifting and other physical activity. The ability to travel to program sites and meetings is required as well.


Minimum qualifications include:

  • Licensed Practical Nursing Diploma and/or certification from an accredited LPN Program or College; Registered LPN licensed in the State of Pennsylvania;
  • Valid driver’s license;
  • Site visits to check medication administration accuracy.
  • Site/individual based trainings on diagnoses/meds/procedures.
  • General Training for all staff on various health related topics.
  • Assure that all pertinent data from past medical history is recorded in the current lifetime medical history.
  • Be able to perform general office work to include writing, typing, filing, use of telephone and communicating with others.
  • Capture all pertinent dates from medical appointment documentation throughout the year and annually update lifetime medical history at the time of the annual planning meeting.
  • Excellent organizational and computer skills;
  • Demonstrates proficiency in reading and writing;
  • Strong presentation skills, interpersonal and problem solving skills and ability to work independently and as part of a team;
  • Minimum of two (2) years’ experience working with individuals with disabilities preferred;
  • Ability to meet both the physical and essential requirements of the position as outlined in job description.
  • Establish contact with hospital when a consumer is hospitalized and assume primary point of contact.Coordinate discharge plans with hospital staff and assure that adequate supports are in place for the individual once hospital discharge has occurred.
  • Assume primary point of contact for pharmacies regarding medication issues.
  • Attend planning meetings to provide any necessary medical information.
  • Assure that the individuals healthcare needs are considered in all aspects of planning.
  • Advocate for the individual’s health care needs.
  • Have the ability to physically assist individuals, administer medications, perform CPR, administer First Aid, push or pull a wheelchair as needed
  • Have the ability to safely operate vehicles: to include passenger cars, vans and vehicles equipped with wheelchair lift
  • From relationships with members of the individual’s team in order to maintain lines of communication in the event of any health-related issues that need to be addressed.
  • On call responsibilities for assessment of medical issues as needed
    Review hospital discharge instructions and ensure that changes are completed to assure the health and safety of the individuals.
  • All other duties as may be required by the Director of Quality and Incident Management.
