Summit Home Health Care is a RN-owned and operated, Joint Commission Accredited, NYS licensed home care agency that services all 5 Boroughs on NYC and Long Island.
We currently have patients that require exceptional Private Duty Nurses (PDNs) to join our team of dedicated professionals.
Experience with Ventilators (vents), Tracheostomies (trachs) and G Tubes is a plus, but we welcome new graduates and nurses looking to expand their clinical skills

Licensed Practical Nurse (LPN) Responsibilities:
- Perform Medication Administration and Treatments per Plan of Care
- Monitor patients and report on their condition
- Accurate documentation in accordance with the provided orders
Licensed Practical Nurse (LPN) Requirements:
- Valid NYS Nursing License
- Government Issued ID, Social Security Card & Work Authorization
- 2 Reference Letters
- Recent Physical
- TB Screening (Quantiferon required)
- Rubella & Rubeola titer lab work
