Job Summary:

We operate our pediatric subacute skilled nursing unit on a LVN model. The Licensed Vocational Nurse (LVN) utilizes knowledge of basic nursing principles, practices, and ethics in order to perform procedures and tasks within their scope of practice, but for children with very special needs.

The LVN is responsible for providing and documenting nursing care, under the supervision of a Registered Nurse, in accordance with developed care plan and physician’s orders for each individual patient and coordinates care among all members of the interdisciplinary healthcare team.

It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support the organization strategic plan, the goals and direction of the quality assurance performance improvement process, and activities.

SPECIFIC FUNCTIONAL RESPONSIBILITY:

  • Assessment and Planning
  • Initiates, updates, and documents age appropriate plan of care in conjunction with interdisciplinary team assessments and goals to ensure quality and continuity of care for patients
  • Performs patient assessments and reassessments and collaborates with the Registered Nurse to develop appropriate plans of care, and nursing interventions
  • Collaborates with the Registered Nurse (RN) to actively assess pain management and implements prescribed medication and/or other alternative methods that are effective for the patient
  • Assists in patient/caregiver teaching with appropriate documentation as assigned
  • Demonstrates knowledge, recognizes the signs of abuse and implements professional response, reporting, and documenting according to laws and organization policies
  • Recognizes and responds competently to acute medical emergencies according to established procedures and scope of practice

Medication and Treatment Safety

  • Applies medication and treatment safety practices by using the six rights to medication and treatment administration: right patient, right medication/treatment, right route, right dose, right time, right approach
  • Administers medication as ordered by physician and documents patient’s response to medications and treatments within scope of practice
  • Communicates with pharmacies on new/refill medications, and checks medication deliveries for accuracy
  • Receives, processes, transcribes and reconciles medication orders timely and accurately
  • Demonstrates knowledge of appropriate techniques and treatment modalities as performed on patients of varying ages, including infant, toddler, adolescent and young adults
  • Demonstrates clinical competency in nursing care to technically dependent patients (such as tracheostomies, gastrostomies, etc.)
  • Recognizes abnormal laboratory values/diagnostic reports and notifies the Registered Nurse in charge and/or physicia

Documentation and Reporting:

  • Measures and documents patients vital information and responds appropriately
  • Observes and documents patients signs and symptoms of illness and therapeutic treatments in medical record
  • Documents and reports changes in patient condition/unusual events utilizing approved reporting system
  • Completes all documentation timely
  • Provides accurate hand-off communication using SBAR reporting to ensure patient safety and continuity of care

Patient Care Area-General

  • Plans, organizes, and prioritizes daily work using sound judgment and following the direction of the Charge Nurse, Nurse Manager, and physician
  • Engages patients with a positive and friendly demeanor, while providing patient and family centered care
  • Ensures and follows infection control policies and current practices
  • Participates in training and orientation of new personnel as assigned
  • Oversees patient care provided by the Certified Nursing Assistant (CNA) during assigned shift

Requirements:

  • Current Licensed Vocational Nurse as issued in State of California
  • Current American Heart Association Basic Life Support (BLS)
  • Pediatric Advance Life Support (PALS) and Advanced Cardiovascular Life Support (ACLS) certification preferred
  • Minimum of two (2) years of pediatric nursing experience in a subacute or acute care hospital; or completion of the hospital’s preceptor program within two months of hire
  • Ability to lift thirty (30) pounds without assistance
  • Ability to communicate effectively in English, both verbally and in writing
  • Knowledge of basic nursing procedures and documentation as applicable to job
  • Must possess good work ethics and a professional image at all times
  • Knowledge of CMS conditions of participation, CDPH state regulations, CCS, and TJC (hospital, nursing care center, and laboratory) accreditation requirements as applicable to job
  • Working knowledge of computer and software applications as applicable to job
  • Possess strong communication, interpersonal and collaboration skills
  • Works responsibly in a team environment as well as independently
