Youth Homes is looking for a Life Skills Coach to join our TAY Program to ensure best-in-class experiences for our youth and families.

About the Role:

Works closely with Transitional Age Youth (TAY) Clinicians to formulate structured community and skill-building activities for Youth Homes’ TAY Full-Service Partnership (FSP) and TAY Aftercare consumers. Develops contacts and works directly with other community programs and resources. Involves and informs youth of options and opportunities available to them by role modeling and coaching around developing self-advocacy skills. Life Skills Coach/Youth Advocate assures TAY program consumers access the full array of available TAY services.

Assisting in the following areas:

Skill-building around making safer and healthier decisions around substance use

Learning individualized adaptive coping skills to manage mental health symptoms better

Assuring foster youth and former foster youth access to county support services

Meeting educational goals through accessing schools, vocational programs, and scholarships

Accessing General Assistance/SSI, when applicable

Acquiring and maintaining housing

Linking with Work Experience Programs and assistance in seeking/maintaining employment

Support in developing self-advocacy, independent living skills, and activities of daily living

Assisting with accessing medical care and navigating medical insurance issues

Assisting with transportation and meeting transportation needs.

You will be successful if you:

Promote and assist in developing, organizing, and facilitating workshops

Provide Medi-Cal billable services such as rehab/case management for 60-75% of the time worked with Youth Homes’ TAY clients as assigned by Clinical Director

Complete all required documentation in an accurate and timely manner

Offer support, referrals, and advocacy as needed for Youth Homes’ TAY Client

Work collaboratively with other members of treatment teams for youth on caseload, including social workers, housing support, medical and educational support, family members, and other caregivers, in addition to members of the Youth Homes TAY Program staff to ensure coordinated care for youth in programs

Provide one-on-one support, skills coaching, guidance, and advocacy to clients within the context of each youth’s treatment team, led by their program clinician

Work with TAY Clinicians to help youth develop and succeed in substance abuse recovery plans in an integrated, team-based treatment manner

Assist in planning and facilitating employment, education, life skills or housing workshops, regular enrichment, skill-building for social networking/engagement workshops and groups.

Attend TAY Program staff meetings and coordinates client care with other TAY Program team members.

Other duties as assigned by the Clinical Director or SVP Clinical Services and Programs
