POSITION TITLE: Licensed Practical Nurse
DEPARTMENT: Nursing
REPORTS TO: Nursing
Supervisor/Nursing Administration.
POSITION SUMMARY:
The
essential functions of the job for a Licensed
Practical Nurse require, and may be amended from time to time at the sole
discretion of Windsor Health Care, LLC to include, but are not limited to the
following:
- Must
state, from an approved NJ Nursing School of Nursing.
- Comply with
health and regulatory standards and guidelines, as applicable.
- Must possess, at a
(1) year of experience in the healthcare field and/or related industry.
- Must be able to
the utmost compassion and dignity to the residents and adhere to the Resident
Bill of Rights.
- Must be able to

RESPONSIBILITIES/ACCOUNTABILITIES:
- Under the
understanding of the principles of nursing and basic physical assessment skills
in the development of and implementation of individualized nursing care plans
to ensure that the needs of the residents are met.
- He/she assists in
operations of the unit per shift, unit level, and assumes responsibilities of a
leadership role as needed.
- Takes an active role in direct resident assessment and care;
- Supervises and coordinates nursing personnel in providing direct resident
- Formulates individualized nursing care plans utilizing the nursing
- Assesses each resident daily and implements a change in the course of
- Assists the physician on rounds and ensures that pertinent information is
- Participates in the training and supervision of nursing personnel;
- Utilizes effectively the general principles of leadership and

noncompliance with facility guidelines as set forth in the Policy andProcedure Manuals;
- Maintains accurate resident care records and documents pertinent data
- Attends and participates productively in the resident care meetings,
- Maintains active licensure;
- Attends all mandated in-services;
- Takes an active role in teaching (formal or informal presentation) with
- Dispenses medication and performs treatments, as requested, and in
- Assures that each resident’s attending physician and family or
resident’s health condition;
- Ensure that residents and families receive the highest quality of service
recognizes the individuals’ needs and rights;
- Adhere to all facility policies and procedures;

- Participate QAPI or facility assessment activities as needed, such as
