VNA of Ohio currenlty has an opening for Manager, Patient Access. This poisition will be responsible for coordinating patient access to care at VNA Ohio, and overseeing day-to-day operations of the local access team. Cultivates current relationships, provides care coordination and collaborates with physicians, hospitals and facilities, health care providers and others in the community as well as supporting and promoting new programs and service lines; works to enhance positive patient and community experience while maintaining an awareness of the business operations objectives for each service line.
Responsibilities:
Principal Responsibilities:
  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with internal and external customers
  • Assists to develop strategies for patient access, working with the clinical leadership team to optimize access to care and balance acceptance of referrals in terms of partnerships and assigned operational metrics
  • Acts as a primary contact for Intake team, Liaisons, and Clinical Managers
  • Works with internal departments to generate ideas and develop strategies that support patients, referral sources and clinician productivity
  • Ensures compliance with regulatory and statutory requirements, and serves as a point of contact for the Ohio Review Choice Demonstration (RCD) initiative team. Provides feedback and education to home care coordinators and liaisons when needed.
  • Participates in the creation of content to promote education of referring providers to regulatory requirements, as needed
  • Monitors performance and evaluates the effectiveness of strategies
  • Analyzes data and prepares reports for initiatives
  • Adheres to the organization’s policy in regards to absenteeism and appearance
Qualifications:
Job Competencies & Minimum Qualifications:
  • LPN or RN required
  • Experience in home health, hospice or private duty healthcare services preferred
  • Excellent verbal and written communication skills
  • Must have outstanding presentation, skills and ability to serve as part of the leadership team of VNA Ohio
  • Experience and competency with use of computers, smart phones, Microsoft Office and clinical software
  • Excellent organizational and interpersonal skills

Working Conditions/Physical Demand:
Office-based with heavy phone and computer use
