Summary/Responsibilities:

Ensuring the completion of the Resident Assessment Instrument (MDS) within federally mandate guidelines. Ensuring that each resident has an individualized plan of care with goals that enhance the quality of life and maximum independence in ADL's. Coordinating care plan conferences to include the interdisciplinary team, the resident, and the family.

Requirements:

QUALIFICATIONS: Active licensed R.N. in the state of Illinois; long-term care experience required; familiarity with MDS and Care Plans preferred; proficient computer skills required; American Association of Nurse Assessment Coordination (AANAC) certification is a plus (if no certification, expectation is that it will be obtained within one year of hire).
