Program Summary:* Personal Care (Individual Support) is not covered under the NC Innovations waiver and is a “hands-on” service for persons with Severe and Persistent Mental illness (SPMI),* a population that is not covered under the NC Innovations waiver. The intent of the service is to teach and assist individuals in carrying out IADLs, such as preparing meals, managing medicines, grocery shopping and managing money, so they can live independently in the community. It is envisioned that the need for the service will “fade” or decrease over time as the individual becomes capable of performing some of these activities more independently. Personal Care under the current NC State Plan emphasizes the need for assistance with Activities of Daily Living (ADLs). Some assistance with Instrumental Activities of Daily Living (IADLs) is covered, but only to the extent linked to ADLs. This service (Personal Care/Individual Support) is coverable under the State Plan, but NC has not included in its approved State Plan.

  • Requirements: Must have a high school degree and minimum of 2 years experience working with ADULTS with mental illness.

Job Responsibilities:

  • Provides psychoeducation as indicated in the Person-Centered Plan
  • To hold a caseload
  • Assists with crisis interventions
  • Assists the Individual Support staff, Program Director, and Clinical director with behavioral and substance abuse treatment interventions
  • Help to assess the child’s needs for comprehensive services (facilitating intakes,
  • Consulting with identified medical (for example, primary care and psychiatric) and non-medical (for example, DSS, school) providers, engaging community and natural supports, and including their input in the person-centered planning process
  • Ensuring linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations
  • Providing and coordinating behavioral health services and other interventions for the client or other family members with other licensed professionals.
  • Monitoring and documenting the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the Person-Centered Plan
  • Monitoring the provision of services and supports
  • Implement CBT, MI, SAMHSA Recovery based models and other evidenced based treatment modality interventions
  • Assessing the outcomes of services and supports
  • Assists with the development of relapse prevention and disease management strategies
  • Communicates the recipient’s progress and the effectiveness of the strategies and interventions as outlined in the Person-Centered Plan

Administrative duties:

§ Conduct initial intakes and help with gathering information of new and existing clients

§ Complete electronic documentation for services offered within 24 hours of the service.

§ Help with entering information in electronic medical records system using the electronic system, IMS Checkpoint

§ Assist with answering phones, faxing, scanning, etc.

§ Other assignments as directed by CEO, Program Manager and Clinical Director.

Educational and Experience Requirements:

The Para Professional must meet one of the following educational qualifications based on the targeted population being served:

a) High school diploma or equivalent

b) The Paraprofessional to recipient ratio shall not exceed 10:1.

c) The Para Professional must be available for monthly group or individual supervision and other job-related trainings

d) Paraprofessionals must be able to deliver services in various environments, such as homes, court, homeless shelters, street locations, and other community settings

e) CPR/First Aid Certification

Job Type: Part-time

Pay: $13.00 - $15.00 per hour
