SUMMARY:

Under the direction and supervision of the Director of Behavioral Health, the social worker in this agency is required to provide both direct services and specialized supportive services to children and adults. The Licensed Integrated Care Clinician is responsible for providing mental and health-related social services, chemical dependency referrals and consultations, on call crisis intervention, and child welfare supportive services as appropriate to meet the needs of Native American clients. This a position in a 5 years Garrett Lee Smith (GLS) grant with option to continue after the grant is completed. Focus of the position is to integrate care between medical and behavioral health services, working closely with both departments, providing brief screens, warm handoff to BHS and follow up treatment case management. Ability to work a variable schedule to meet the patient and program needs.

MAJOR DUTIES AND RESPONSIBILITIES:

Duties of this position include, but are not limited to the following:

· Assist the project director to ensure the project purpose, goal, objectives, requirements, outcomes, and timelines are achieved and reported the Garrett Lee Smith (GLS) grant.

· Work with the GLS project team to develop, coordinate, and monitor the delivery of GLS services in accordance with the GLS implementation plan, evaluation plan and QI plan.

· Deliver the following GLS services in the assigned medical department location:

o Oversee patient screening to ensure that universal screening procedures are implemented by medical department staff members during the vital routine

o Complete informed consents

o Evaluate positive screening results, including suicide risk evaluations

o Deliver education to the youth and family

o Deliver supportive counseling when needed

o Make referrals (warm hand-off when needed)

o Contact patients who miss their follow up appointments in the medical department, BHS department, and/or other speciality treatment provider to engage them in GLS Services

o Follow-up with youth, family members, friends and other agency representatives when referred to emergency services or after a suicide completion

· Duties include interviewing, evaluation, ongoing risk assessments, identification of and accessing necessary resources for clients, recommendations for assigned clients based on the individual’s medical, psychological, social and chemical dependency/abuse needs.

· Document all informed consents, evaluations, treatment actions, referrals, and follow-up contacts in ScreenDox and in the NextGen EHR.

· Provide technical assistance to GLS project team to solve problems with screening, evaluation, intervention, referral to treatment, and follow-up procedures

· Attend project-related meetings with the GLS project team, other RSBCIHI staff members, and the Integrated Care Committee

· Other duties as assigned

MINIMUM QUALIFICATIONS

Education: Competition of Master’s degree from an accredited school that fulfills the academic requirements as a Licensed as a LCSW, LMFT or LPCC.

Licensed: Licensed as a LCSW, LMFT or LPCC. Must have current and valid State of California license.

Experience: Experience working with American Indian/Alaska Native populations preferred. Experience working with suicide risk evaluation, education, and supportive counseling preferred

Knowledge: Demonstrate understanding of Native American communities, knowledge of family systems, and substance abuse12-step support systems. Social worker will be familiar with principles and techniques of social work and of the function of public social service agencies; socioeconomic and cultural factors which promote stable family life, and an understanding of the factors that affect family security; physical and mental illnesses and their impact on community organizations and the services they provide.

Skill: Superior skill in working with other professionals such as The Department of Children’s Services, physicians, psychologists, nurses, lawyers, judges, educators and other related professionals such as Indian Child Welfare, and Indian Child and Family Services. Be able to apply the principles of general and/or child psychology and family relationships. Evaluate personal and psychological factors in the client and/or family’s situation. Possess clear and effective verbal and written communication skills.

Ability to: Establish rapport with clients and their families effectively apply casework, knowledge and skills, Work constructively within RSBCIHI organization and community setting, effectively utilizing appropriate resources and services. Suspend personal bias and apply objectivity and social work ethics. Maintain effective relationships with other employees, professionals and the public. Function effectively as a clinician, work independently when required yet function as a team member with other agency staff. Must be able to work with the Indian Community and be sensitive to the Indian culture and its needs.
