About the role:

We are looking for a Licensed Mental Health Therapist to work in collaboration with the multi-disciplinary team to provide brief /short term behavioral health interventions and counseling to home care patients and their families. Develop and facilitation of support groups to address various topic related to patients with serious, complex illness and behavioral health diagnosis.

This position will report to Behavioral Health Clinical Operations Manager

  • Provide outpatient behavioral treatment services to individuals with mental health disorders, substance use disorders, and/or co-occurring disorders utilizing evidence-based therapies (CBT, DBT, BA, CPT, etc.)
  • Crisis treatment and intervention services
  • Perform initial, comprehensive behavioral health assessment and screenings (GAD7, PHQ9, CAGE, MoCA, C-SSRS, etc)

on patients, develop, and implementation of treatment care plan

  • Conduct first line mental health assessment. Formulate basic impressions re. cognition (thought process, thought content, thought flow), mood (euthymic, depressed, anxious etc)/affect (broad, constricted flat etc)
  • Assist patients and their families/caregivers in understanding, accepting, and development of coping strategies related to their medical and behavioral health diagnosis
  • Link patients and family members/caregivers to appropriate community resources
  • Consults with multi-disciplinary team for the purpose of assisting them to understand significant social, emotional, and environmental factors related to the patient’s health problems
  • Provide counseling services for end-of-life
  • Prepare notes for each visit/phone call and incorporate in electronic medical record
  • Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies
  • Other job-related duties as required.

Candidate requirements:

  • Master’s Degree in Social Work (MSW) or Mental Health Counseling from a recognized college and possesses a valid NYS license - (LMSW, LCSW, LMHC)
  • Maintain licensure and all required CEUs
  • Two years of experience in providing mental health counseling where a multidisciplinary team approach to treatment was utilized. Experience in providing care in a medical setting (Primary care practice, clinic, or hospital) preferred
  • Working knowledge of evidence-based psychosocial treatment and brief behavioral interventions for common mental health disorders (motivational interviewing, problem solving treatment and behavioral activation)
  • Experience in delivering home based counseling and support group facilitation is preferred
  • Specialized knowledge, training, experience with geriatric-specific issues (end of life, grief and loss, effects of chronic medical conditions, etc.).
  • Proficient in MS Office, electric medical record documentation, and patient management software
  • Excellent communication and people skills
  • Exceptional organizational and leadership skills
  • Aptitude in resolving issues and conflicts
  • Valid NYS driver’s license
