About the Position:

Under the general direction of the Nursing Supervisor, the Registered Nurse is primarily responsible for the individuals' healthcare. To help the individuals maintain and improve their health status by providing oversight for medical conditions and treatments. To maintain adherence to medical protocols outlined by OPWDD regulations and HASC Center Policy and Procedures. Represent the Agency by promoting and modeling the mission and core values of HASC Center.

The HASC Center mission is to advocate for, educate and assist individuals with developmental and intellectual challenges to make life choices, exercise independence and develop responsibility. This is accomplished by providing an array of individual and family supports through a dedicated team of professionals and support staff.

Responsibilities:

• Manage individuals' medication including ordering medication, maintaining a supply of medication, notifying appropriate parties of any medication changes, and ensuring medication administration is accurately documented.
• Provide AMAP training and/or recertification as well as any person-centered medical training needed to ensure the health and safety of individuals being supported to DSPs.
• Serve as the liaison with medical providers. Schedule appointments and attend medical appointments as necessary. Ensure follow-up care as ordered by the physician.
• Review incoming physician summaries for instructions, medication changes and treatment orders and implement all changes.
• Ensures that all emergency room/hospital discharge instructions are followed up and documented
• Manage notifications to proper authorities detailing changes in an individual's medical condition.
• Implement, monitor, and revise individuals' medical orders and protocols.
• Directly observe individuals on an ongoing and as-needed basis.
• Maintain appropriate documentation, e.g., weight, vital signs, results of a medical appointment.
• Monitor and maintain regulatory compliance, agency policies, procedures, and standards regarding the individuals' health care.
• Serve as the liaison with other care providers including but not limited to the day habilitation program, medical care providers to maintain appropriate communication, and ensure consistency of medical services.
• Ensure all employees are effectively trained. Facilitate training programs as needed.
• Demonstrate good nursing judgment in determining appropriate care.
• Facilitate and actively participate in team meetings and staff meetings.
• Functions as a member of the interdisciplinary team contributing to the development of individual program plans.
• Monitor staff to assure compliance with accepted infection control practices.

Education and Experience:

· Preferred Education: Bachelor's Degree, and NYS nursing license. · Preferred Experience: Three years in a nursing setting.

· Minimum Education: Associates Degree and NYS nursing license required. · Minimum Experience: At least one year of nursing experience.
