Job Title: Nurse Case manager
Job Summary
Our ideal candidate has a strong working knowledge of remote-patient monitoring (RPM) and remote care management (PCM, CCM, TCM) principles. A unique remote work opportunity, this role provides the chance to serve patients by proactively monitoring, educating, and coaching patients on a plan for better health. Early intervention through virtual care modalities reduces risk for emergent care and/or hospital admission/re-admissions. As a leader of the care team interacting with patients and allied health professionals, this position will have the opportunity to provide valuable feedback on best practices.
Duties/Responsibilities
- Provide preventive health and disease management education
- Evaluate patient trends and provide interpretive feedback
- Schedule and perform video calls and/or phone calls with patients and their caregivers
- Define health goals in conjunction with providers and patients
- Manage patient messages and respond
- Influence behavior for positive outcomes
- Triage alerts for relevance and respond with urgent action if needed
- Document findings and relay messages to physician
- Provide in-clinic support for patient engagement and enrollment as needed
- Lead team of care technicians
- Develop standard operating procedures (SOPs) and best practices for care delivery
- Guide team with clinical expertise in patient data interpretation
- May be required to perform other duties as assigned
- Weekend and/or holiday on-call rotation as needed
Required Experience, Qualifications, and Skills
- Unrestricted RN license in Compact States, and MI, NY, CA, or PA or the ability to obtain licensure in these states
-Case Managment experience preferred
- Strong clinical background with 5+ years nursing experience.
- Experience and/or certification in specialty areas, such as cardiology, renal disease, or diabetes, is desirable.
- Strong interpersonal and communication skills. Comfortable communicating with patients and caregivers, physicians, and other clinicians.
- Critical thinking skills to address top-level concerns and leverage all available data to determine the best path forward
- Strong personal and professional judgement
- Ability to respond quickly to shifting realities and adjust initiatives and priorities accordingly
- Demonstrate leadership ability, experience leading teams, projects, and client implementations
- Capable of working independently and demonstrates self-motivation
- Excellent documentation skills
- Adheres to the CRMS policy; exudes confidence and positive body language
- Understanding of all HIPAA regulations and the need to strictly adhere to them
- Proficiency with MS Office Suite including managing MS SharePoint and Excel
- Proficiency with internal messaging platforms including Microsoft Teams
- Proficiency with virtual meeting platforms including Zoom, WebEx, Skype, and Microsoft Teams
Physical Requirements
- Prolonged periods sitting at a desk and working on a computer
- Viewing monitors for long periods of time which may cause eye strain
- Freedom to get up and move from workspace as needed
- Ability to use various office equipment such as copiers, fax machines, phones
