Bear Mountain Healthcare at Andover has immediate openings for a Case Manager looking to work in our Long Term Care Facility in Andover MA.

With 135 beds and various amenities, we bring a new level of comfort to the care received. We provide post-acute services, rehabilitative services, skilled nursing and short-term/long-term care.
Case Manager Provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy. The Clinical Case Manager is responsible for facilitating, reviewing and preparing interdisciplinary plans as well as assuring comprehensive progress reports are completed and provided to the payor as required; Also serves as liaison between patient, physician, care team members, payor, and the discharge planner/social worker by monitoring and communicating patient's progress and cost evaluation and assisting with transition to the next level of care.
