Salary: $92,250.50 - $112,398.42 annual (DOE)

Position Summary:

Under general supervision, provides information and instruction to individuals and families in health education and disease prevention. The incumbent is responsible for coordinating all patient education activities in the Tohono O'odham Nation Sells Hospital clinics, including employee in-service, community health activities, and coordination with other agencies and programs.

Scope of Work:

This position is located in the Tohono O'odham Nation Sells Hospital, including San Xavier Health Center, Santa Rosa Health Center, and San Simon Health Center in the Ambulatory Care/Nursing areas. The position aims to plan, develop, implement, evaluate, and coordinate patient/staff education programs and community health activities for Tohono O'odham Health Care. The incumbent is under the general supervision of the Supervisory Nurse Specialist Ambulatory Care. Incumbent may be delegated Quality Assurance duties as determined by the Supervisory Nurse Specialist or Facility Director (for Health Centers only).

Essential Duties and Responsibilities: (Depending on the area of assignment, an incumbent may not be required to perform some of the duties listed below):

  • Responsible for coordinating computer tracking programs present in place for women's health, immunizations, and other services as required.
  • Develop tracking and educational curriculum for patients with diabetes.
  • Assist with coordinating quality assurance activities in the clinic to include compliance with Joint Commission (JC), Health Care Finance Administration (HCFA), Government Performance and Results Act (GPRA), and various other regulations.
  • Organize and maintain clinic quality assurance programs to comply with JC, HCFA, GPRA, and other regulations.
  • Organize, maintain and coordinate patient education program with components to include Prenatal, Well Child, Chronic Disease Care, Women's Health, Employee Health.
  • Assists community members and health field personnel to assess and plan for needed health-related services.
  • Teaches home care material and instruct community members on subjects related to individual and community welfare.
  • Participates in programs to safeguard children's health, including conferences, school health programs, group instruction for parents, and immunization programs.
  • Instructs individuals and families in health education and disease prevention.
  • Maintains professional knowledge by conducting research, attending seminars, educational workshops, classes, and conferences; conferring with contracting agencies and related organizations representatives.
  • Performs other job-related duties as assigned.

Knowledge, Skills, and Abilities:

  • Knowledge of Tohono O'odham traditions, language, history, geography, and culture.
  • Knowledge of applicable federal, state, tribal laws, regulations, and requirements.
  • Knowledge of Community Health Nursing principles.
  • Knowledge of basic and advanced scientific and psycho-social nursing theory, concepts, principles, and techniques.
  • Knowledge of curriculum development and teaching plans.
  • Thorough knowledge of diabetes, the disease process, and complications.
  • Thorough knowledge of perinatal, well-child, and women's health issues is required.
  • Knowledge of common primary care health problems such as arthritis, asthma, STDs, hypertension, and chemical dependency.
  • Thorough understanding of primary care systems.
  • Ability to work in a cross-cultural environment.
  • Must be current in state-of-the-art health promotion/disease prevention knowledge.
  • Knowledge of JC/QA Standards of Ambulatory Health Care facilities.
  • Skill in analyzing problems, projecting consequences, identifying solutions, and implementing recommendations.
  • Skill in providing superior customer service to external and internal customers.
  • Skill in operating various word-processing, spreadsheets, and database software programs.
  • Ability to operate medical equipment to include: blood pressure, Accu-check and glucose machines, sphygmomanometer, and other related equipment.
  • Ability to carry out instructions furnished in verbal or written format.
  • Ability to communicate efficiently and effectively verbally and in writing.
  • Ability to create and present effective speeches and presentations.
  • Ability to exercise independent judgment.
  • Ability to establish and maintain positive and effective working relationships with other employees and the general public.
  • Ability to maintain privileged, confidential information.
  • Ability to work extended hours and various work schedules.
