Job Summary

Public Health Nurses practice in a variety of settings. They participate in committees and help develop policies, and may even assist on grants. 1. Performs home visitation in accordance with the required evidence-based NFP model.
2. Develops therapeutic relationships with women and their families in a home visitation community environment.
3. Assesses physical, emotional, social, and environmental needs of women and their families as they relate to the NFP domains.
4. Provides education, support, and referral resources in assisting women and their families in attaining their targeted goals.
5. Consults and collaborates with other professionals involved in providing services to women and their families.
6. Actively engages in professional development and skill building to meet all NFP nurse home visitor competency requirements.
7. Utilizes the computer for long-distance learning.

This position may be eligible for student loan repayment via the Nurse Corps Loan Repayment Program.

Primary Duties & Responsibilities
1. Provide home visits to first-time Mothers and their families eligible for the NFP Program.
2. Adhere to nursing process and the NFP model of home visitation.
3. Carry a case load of 25 or more clients.
4. Support NFP policies, procedures, Visit-to-Visit Guidelines, and standards of NFP and Calhoun County Public Health Department.
5. Develop therapeutic relationships, utilizes concepts of reflection and motivational interviewing with women and their families in a home visiting environment.
6. Perform home visiting and telehealth service delivery in accordance with the NFP model and Visit-to-Visit Guidelines.
7. Follow nursing process in implementing NFP into nursing practice.
8. Assess physical, emotional, social, and environmental strengths and risks of women and their families as they relate to the NFP domains.
9. Assist women and their families in establishing goals and outcomes.
10. Provide education, support, and referral resources in assisting women and their families in attaining their targeted goals.
11. Consult and collaborate with other professionals involved in providing services to women and families.
12. Formulate nursing diagnosis based on nursing assessment and client goals.
13. Evaluate client progress toward stated goals and NFP outcomes.
14. Plan home visits in accordance with client goals and NFP outcomes.
15. Actively engage in professional development to meet all NFP nurse home visitor and licensure competency requirements.
16. Participate in quality improvement efforts.
17. Maintain confidentiality.
18. Document appropriately.

Job Qualifications
1. BSN required.
2. Current RN license in good standing required.
3. Current CPR required.
4. Two years recent experience in maternal/child health, public health, home visiting, or mental/behavioral nursing preferred.
5. Home visiting experience preferred.
6. Excellent communication and interpersonal skills necessary to provide courteous and accurate information often under stressful situations for families.
7. Good computer skills to communicate regularly via email, to collect data on electronic forms or formatted spreadsheets, and for long-distance learning.
8. Mental ability to frequently handle simultaneous projects requiring concentration and attention to detail with frequent distractions due to people and/or telephone calls.
9. Occasionally required to climb and/or kneel and periodically required to stoop or bend to accommodate different working positions.
10. Frequently required to walk and/or stand for extended periods of time, perform repetitive movement, and/or work in confined areas.
11. Valid Michigan driver’s license, an acceptable driving record, and insured personal vehicle required.
12. Must be able to work a flexible schedule including some evenings and weekends (per agency requirements).
13. Able to travel by air to Denver, CO for all face-to-face education sessions.

Flexible Scheduling & Telecommuting Options Depending on Position & Department

Paid Time Off:
Start through fourth years – 144 Hours (3.6 weeks based on 40-hour work week)
Fifth through ninth years – 184 Hours (4.6 weeks)
Tenth through fourteenth years – 224 Hours (5.6 weeks)
Fifteen and Subsequent years – 264 Hours (6.6 weeks)
