Job Summary & Responsibilities

Job Summary

This position functions as a member of the health care team and provides care coordination, and management to patients as a member of the care delivery team; Activities include assessment, triage, identification, planning, implementation of an individualized plan of care, evaluation of outcomes and supporting Brown & Toland. Nursing care is provided in accordance with the provisions of the California State Board.

Essential Functions

1. Function as part of a practice team to provide patient care.
a. Use patient centered care concepts in delivering patient care and performing related activities
b. Proactively participate in huddles to review daily schedule tasks, and prepare for the day; prepare patient agenda including verifying procedures, review and update Health Maintenance Plan, overdue services, review archived charts, obtain additional information to prepare patients for their appointments.
c. Provide patient care services in accordance with the provisions of the state requirements: services include injections, urine analysis, EKG, wound care, and ear lavage; assists providers with examinations, procedures and other processes related to direct patient care.
d. Prepares and administers vaccines and medications as prescribed by the provider; asks screening questions, identifies risks and contraindications, questions and observes for appropriate responses; document and processes prescription requests and refills per provider order.
e. Maintains patient records to ensure an accurate record of the patients’ medical treatment.
f. Support and triage patients’ telephone calls and portal message; independently assesses patients health status based upon presenting symptoms and provide advice following defined protocols; determines the urgency for care and directs patient to most appropriate level of care under the direction of a provider
g. Provide overview of additional follow-up care needed; coordinate, track and monitor diagnostic test and specialists’ appointments under the supervision of a provider; provides patients and their family related education, counseling, ongoing review and monitoring regarding diagnostic tests (i.e., INR results in Anticoagulation Clinic) in collaboration with the provider; this includes coordinating care for After Hours, Other Annual Wellness Visits, Home Visits and other practice initiatives.
h. Assists in the management of patient in care homes, between visits and transitions, i.e. post discharge from an admission.
i. Provides health coaching activities across the continuum of care to facilitate and promote high quality, cost effective outcomes, focused on the patients’ orientation, self management decision support and aims to minimize any fragmentation of health care delivery.
j. In collaboration with providers, provides extensive health education, counseling and advice to patients and families on topics such as life style changes, cholesterol lowering diets, diabetic control etc. Effectively assesses patient educational needs, prioritizes patients learning needs, identifies barriers to learning, develops teaching plans, and uses various educational tools to activate the non-compliant patient.
k. Masters the use of electronic tools to identify and target needs of risk populations; lead population management efforts with care providers staff.
l. Assist in quality improvement, auditing, safety, and compliance measures of the organization
m. Serve as a resource to patients and providers; maintains up to date information on community resources, health coaching literature or recommended screenings and community based programs.
n. Participate in professional development activities and maintains professional affiliations.

Education

  • Graduate of approved LVN Program.

Experience

  • 1. Minimum of one year experience with direct patient care in out-patient setting 2. Experience with Electronic Medical Records is preferred.
  • Maintains a current and good standing state license as a LVN 2. Knowledge of nursing practices, techniques and methods applied to patients
