About the role:
We are looking for a Nurse Practitioner to manage a panel of high-complexity patients performing home visits. The Nurse Practitioner will be responsible for providing chronic disease management supporting primary care physicians in the patient’s home. The NP will contribute to the plan of care making recommendations based on caring for patient in their environment. This position will report to the Advanced Practice Provider Operations Manager.
Responsibilities:
- Assesses patient health by interviewing patients; performing physical examinations; obtaining, updating, and reviewing medical histories.
- Complete comprehensive, accurate and thorough review of the assigned member population, including timely completion and submission of all required encounter documentation in Electronic Medical Record (EMR)
- Assist with the management of patient care on a longitudinal basis, with the primary objective of delivering proactive chronic disease care and significantly reducing avoidable hospitalizations
- Develop and implement clinical plans of care for adult patients facing chronic and complex conditions (comorbid medical, comorbid mental health diagnosis)
- Working closely with other team members to ensure quality care is delivered including closing gaps in care
- Coordinate with patient’s other providers including specialists and health plan clinical team (case managers) to ensure all are working together with the patients towards the same goals and to coordinate optimal care and resources
- Educate the member and their family about medication usage, side effects, illness progression, diet, nutrition, medical adherence, crisis anticipation and prevention
- Provides continuity of care by developing and implementing patient management plans
- Prescribe medications as permitted
- Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies
- Other job-related duties as required.
Candidate requirements:
- Nurse Practitioner (NP) accredited program. Current, unrestricted NY State PA or NP license
- Two years of clinical NP experience required in a relevant field
- Experience in home-based care preferred
- Ability to provide clinical advice, guidance, and support to professional staff in areas of specialty
- Excellent knowledge of standards and regulations for the clinical field
- Excellent communication and people skills
- Exceptional organizational and leadership skills
- Aptitude in resolving issues and conflicts
- Valid NYS driver’s license
