TITLE: OUTREACH SPECIALIST

REPORTS TO: Program Director CMHC

Department: Community Mental Health Treatment Centers

SUMMARY: We are seeking an Outreach Specialist for our Home and Community Mental Health Treatment Centers located in Brooklyn. Services are provided on and off site and community settings. The Outreach Specialist will facilitate and co-facilitate outreach activities, actively support clients with case management and referral questions and work toward ensuring the development of program and community partnerships.

JOB SUMMARY:

· Conduct phone and field outreach to help facilitate and develop strong connections with community agencies and service providers

· Conduct routine monitoring of patient experiences through assessments and engagement.

· Assist in linking clients to services that will assist them toward meeting their treatment goals.

· Play an active role in the development and promotion of the community advocacy board.

· Educate clients, consumers, community members about the agencies services and benefits.

· Document all client contacts within the electronic health records within a timely fashion.

· Conduct community outreach events, including fairs, presentations, workshops and trainings as needed to develop and further enhance community partnerships.

· Conduct additional case management or outreach tasks as assigned.

· Be flexible with the schedule as needed.

QUALIFICATIONS:

  • Peer Certification
  • High School Diploma
  • Must have experienced mental health concerns themselves and/or have supported family members through their mental health experiences
  • Commitment to Recovery-Oriented Practice with at least 1 year experience in community outreach.
  • Positive attitude and professional demeanor
  • Ability to complete work independently as well as in collaboration with team members
  • Must be team oriented with a willingness to be flexible and helpful
  • Ability to travel daily into any area of Brooklyn and vicinity
  • Effective written and oral communication skills
  • Computer proficiency in Microsoft Office a MUST
  • Ability to learn Electronic Health Record System
  • Highly organized, motivated self-starter
  • Excellent time management skills.
  • High School Diploma or GED
  • Spanish Speaking A PLUS

JOB TYPE: FULL-TIME

SALARY: $45,000-$55,000 (Based on experience and qualifications)
