PATH is seeking candidates with a strong understanding of outreach, engagement, and case management practices to join our Metro LA programs as an Outreach Coordinator.
ABOUT OUR STREET OUTREACH PROGRAMS
Our Street Outreach programs work to engage individuals who are currently homeless and living on the streets in an effort to provide and link them with services that will ultimately lead them to stabilize in housing long-term.
WHAT IS AN OUTREACH COORDINATOR?
The Outreach Coordinator works with PATH program leadership and community Coordinated Entry System (CES) partners to ensure full geographic coverage of homeless outreach services within the LA Metro Service Planning Area (SPA). Responsibilities of the Outreach Coordinator include:
- Working with the CES Coordinator to develop strategies to increase awareness and effective use of the CES by service providers and the community
- Responding to outreach requests and sending follow-up information to the requestor and supervisor within the required timeframes
- Representing PATH at community and provider meetings
- Engaging with and assessing the capabilities of community groups and other non-professional outreach teams
- Working with street outreach teams and first responders to establish best practices for identifying and working with individuals with a history of being disconnected from community resources
- Identifying potentially underserved geographic areas and hotspots to target for coordinated outreach activities
- Collaborating with program leadership to support building the capacity and developing the outreach teams
- Facilitating outreach meetings with teams serving the SPA
- Maintaining complete and accurate documentation of services, objectives, and outcomes
WHAT YOU BRING
We’re looking for candidates that:
- Maintain and execute confidential information according to HIPPA and all other applicable standards
- Take a non-judgmental, respectful approach when working with individuals with urgent and multiple case management and health needs
- Work just as well independently as they do on a team
- Exercise mature judgement, and are highly motivated, self-starting and proactive
- Are excellent at communicating, whether in writing or verbally
- Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment
- Demonstrate strong interpersonal skills, including the ability to interact and build successful collaborative relationships with a diverse range of individuals at all levels of the the organization, both internally and externally
PREFERRED QUALIFICATIONS
An excellent candidate for this role will:
- Have experience with HMIS
MINIMUM QUALIFICATIONS
To be considered for this position, we require all candidates:
- Hold a Bachelor's Degree in a related field OR have an equivalent combination of education and/or experience
- Have at least 2 years of case management experience in homeless services or working with highly vulnerable populations, particularly those dealing health, mental health, and substance abuse changes.
- Demonstrate knowledge of and/or experience with advanced and evidence-based outreach and case management interventions including but not limited to:
- Engagement and rapport building with hard-to-reach clients
- Harm Reduction strategies
- Motivational Interviewing
- Housing First practices
- Non-violent crisis intervention techniques
- Be proficient with computers and technologies used for work outputs including:
- Microsoft Office Software
- Database systems such as HMS
MANDATORY REQUIREMENTS
Regardless of qualifications, the Outreach Coordinator must:
- Be able and willing to provide in-home and community-based services on a regular basis
- Be able and willing to work flexible hours which may include evenings or weekends
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Provide proof of full COVID-19 vaccination
- Successfully complete the following as a condition of hire:
- Tuberculosis Test
- Background Screening
- Drug Test
DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:
- Have reliable transportation
- A valid driver’s license
- Proof of insurance and ownership for personal vehicles used during work duties
- The ability to qualify for PATH's insurance coverage
COVID-19 VACCINATION
Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.
FLEXIBLE WORK
PATH is a California employer. Therefore, all regular and customary work must be performed within the state. Approval for work outside of the office, does not establish approval for work to be conducted outside of California.
