Description


1. Clinical Practice
a. Elicits and records physical and mental health status, psychosocial history, including review of bodily systems.
b. Plans, teaches, promotes and manages physical and mental health care in a continuous program.
c. Identifies health problems and learning needs of the care recipient.
d. Performs office examinations such as physicals, follow-up focused exams, post-operative exams, etc. and records findings of such. Assists in the pre-operative evaluation of patients, including reviewing available test results and laboratory values, and following up on testing and consultations ordered by the Vascular Surgeons.
e. Assesses findings of history, review of systems, physical examination and diagnostic tests and formulates a diagnosis prior to implementing a treatment regimen.
f. Initiates appropriate diagnostic tests to screen or evaluate the patient's current health status.
g. Implements and manages treatment regimens and administers, prescribes, dispenses and procures pharmacological agents.
h. Performs admission history and physicals.
i. Conducts daily hospital rounds.
j. Formulates and implements treatment and therapeutic plans for inpatients.
k. Arranges and coordinates discharge plan, starting at the time of admission, and including follow-up appointments, tests and arrangements such as skilled rehabilitation, long term care, etc.
l. Reviews medications at time of admission and discharge, reconciling home medications with any changes made during hospitalization, and including patient education concerning effects, side effects, etc.
m. Performs First Assist duties in surgery.
n. Initiates appropriate emergency treatment in life-threatening or unusual situations in order to stabilize the care recipient.
o. Performs diagnostic testing within the hospital setting as indicated.
p. Continually monitors and ensures that continuity of care is uninterrupted between the physicians and the physician assistant.
q. Performs minor surgical procedures.
r. Documents all patient care activities accurately and in a timely manner as per policy and procedure and according to regulatory mandates
s. Writes orders in the recovery room, talks with the patient and family.
t. Performs privileged procedures safely and with informed consent, as indicated.
2. Professional Development - Ensures improvement in practice setting by assuming responsibility for self-development and life-long learning. Provides guidance to others in the practice.
. Collaborates in the development and implementation of educational plans for identified learning needs of clinical colleagues
3. Press-Ganey
. Works effectively and cooperatively with other members of the team to identify opportunities for improvement in Press-Ganey results and actively participates in recommending plans of action to address any identified issues
a. Serves as a liaison between the Vascular Surgeons and/or practice and pharmaceutical, equipment and DME representatives.
4. WDH Core Measures and Pathway Adherence.
. Works effectively and cooperatively with other members of the team to identify opportunities for improving processes related to the WDH Core Measures and Pathway Adherence. Actively participates in recommending plans of action to address any identified issues.
5. Other Duties
. Provides other functions common to the Physician Assistant role for which the PA-C is educationally and experientially prepared.


Qualifications


  • Experience Minimum Required
  • At least 2 years of inpatient or combined inpatient/outpatient care of patients aged 15 to geriatric age.
