PURPOSE: Traveling throughout the Phoenix Metro Area to provide support to the company officers, directors, managers, and members by completing monitoring in compliance with all applicable regulations and policies.

MINIMUM QUALIFICATIONS

• Education and/or experience in customer service and in the field of developmental
• Must be at least 18 years of
• Current valid AZ Driver's License
• Satisfactory background
• Able to acquire a DPS fingerprint
• Familiarity with documentation, scanning and mobile device
• Bilingual preferred

ESSENTIAL JOB FUNCTIONS

• Able to complete attendant care monitoring following the required schedule (5 days of initial service, 30 days, 60 days if needed, 90 days thereafter. Every staff providing ATC also will be monitored after 90 days and annually)
• Demonstrate respect and promote
• Able to contact family members to schedule required visits and to answer questions as needed.
• Must have reliable transportation and the ability to visit members in their homes.
• Paid per visit and may qualify for mileage bonus
• Able to complete required electronic forms and disseminate to all required personnel.
• Work as a team member with others to assist with their duties as necessary
• Follow all company and State policies and procedures including, but not limited to:
  • Reporting to work on time as
  • Refraining from excessive
  • Obtaining prior approval for desired time
  • Maintain company property and
  • Maintain all records, logs, and other documentation as required by ISP, company and funding source policy and
  • Ensure that a clean and safe environment free from abuse, intimidation and harassment is
  • Maintain all licenses, training and certifications as required by funding source and/or Company policies and procedures.
