SUMMARY: Assist individuals currently or previously living in the encampment along the San Lorenzo River in Santa Cruz in acquiring and sustaining housing by offering housing navigation and case management. Provide case management services and assistance with the development of Housing Pathways Scholarship Program plans tailored to their rehousing goals and needs. Services provided include planning, monitoring, coordinating with other agencies and other activities as needed. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential job functions.

I. ESSENTIAL DUTIES AND RESPONSIBILITIES are outlined below. Other duties and responsibilities may be assigned.

A. Case Management and Scholarship Services

  • Providing early identification and intervention for behaviors that may jeopardize housing.
  • Ensure that services provided are culturally appropriate and translation services are available and utilized based on the individual needs of each participant or participant household.
  • Document three types of attempts at engagement (i.e. Phone call, email, location search) for all received client referrals within 30 days of receiving the referral.
  • Conduct strength-based assessments for all participants to determine participants’ current strengths, desires and aspirations, and past personal, social, and environmental resources that can support collaborative rehousing efforts.
  • Provide peer support groups for enrollees to support the development of individual Scholarship plans.
  • Ensure the scholarship plan will best utilize the maximum available rehousing financial assistance per participant household.
  • Document rehousing support and financial agreements with participants and maintain copies of those agreements within individual participant files in HMIS.
  • Develop and document scholarship financial investment plans as part of strength-based recovery or goals plans including rehousing and flexible financial assistance budgeting.
  • Coaching on developing and maintaining key relationships with landlords/property managers with a goal of fostering successful tenancy.
  • Advocacy and linkage with community resources.
  • Assisting with benefits advocacy, including assistance with obtaining identification and documentation for SSI eligibility and supporting the SSI application process. Such service can be subcontracted out to retain the needed specialized skillset.
  • Coordinating with the tenant to review, update and modify their housing support, scholarship and crisis plan on a regular basis to reflect current needs and address existing or recurring housing retention barriers.
  • Providing independent living and life skills including assistance with and training on budgeting, including financial literacy and connection to community resources.

B. Housing Navigation Services

  • Assists individuals in obtaining housing including housing search, and assistance with housing related paperwork such as rental applications and housing voucher applications.
  • Conducts a housing needs assessment and individual-driven housing searches, focusing on the preferences of the individual.
  • Assists individuals with transportation to housing appointments and other housing related needs.
  • Researches, creates and maintains a list of community housing resources including available rental units in Santa Cruz County and other services that may assist an individual when housed (information on public transportation passes, SNAP program, furniture donation).

C. Teamwork and Collaboration

  • Maintain cooperative and professional working relationships and communications.
  • Give input to management on improving the quality of programs and assists in monitoring program effectiveness and conduct tenant satisfaction surveys.
  • In a culturally competent manner, will provide services to all individuals without regard to race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, and handicap or payment status.

II. QUALIFICATION REQUIREMENTS: Knowledge, skill, or ability required to perform the job.

A. Education and/or Experience

  • B.A. or B.S. in social work or a related field; or two years' experience working in social services.
  • Understanding of the Santa Cruz County Housing/Rental market.
  • Previous experience assisting individuals experiencing suffering from chronic homelessness preferred.

Job Type: Full-time

Pay: $22.00 - $26.00 per hour

Schedule:

  • 8 hour shift
  • Monday to Friday
