Connected Health Care is searching for a Patient Care Coordinator to serve in a hospital in Santa Fe, NM. Our client is dedicated to enhancing the health and well-being of every person they serve.

JOB DESCRIPTION:
Responsible for performing all tasks necessary to increase physician efficiency and improve patient experience. This includes being the primary contact at the clinic for the patient for any questions of either a clinical or administrative nature (coordinating clinical calls with the physician), as well as coordinating the physician's schedule (both OR and clinic, as applicable), answering telephone calls, returning messages, and scheduling follow up tests and referrals for patients.
This position requires an excellent working relationship with the physician and the ability to treat patients with respect, privacy, and kindness always, even in times of stress.
MINIMUM QUALIFICATIONS:
Education: Nursing Degree required
Experience: 1+ years of experience preferred.
Licenses/Certifications: Current RN licensure from the State of Practice or Compact license allowed.
