STATUS OF POSITION: Part-time

Position Summary

Patient Concierge is responsible for ensuring patients access their care in a seamless and uncomplicated manner. Perform duties necessary to provide safe, courteous, and efficient transport of patients to and from Community Health Net practice sites & appointments.

Essential Duties and Responsibilities

• Provide concierge-type support by assisting with patient transportation/escort throughout Community Health Net facilities.
• Exhibit strong customer service, effective communication, and critical thinking skills in response to patient and Community Health Net Department inquiries and requests.
• Facilitate an exceptional patient experience while providing concierge-type patient support.
• Maintain clean, professional, and safe environment within Community Health Net Care Van.
• Offer general health education opportunities to Community Health Net patients during patient interaction.
• Track and request routine maintenance needs of Community Health Net Care Van according to the Care Van owner’s manual and specifications.
• Utilize assigned patient transport schedule.
• Ensure transportation tracking is current and accurate.
• Perform other job-related duties as directed.

Requirements/Qualifications

• COVID-19 vaccination required.
• GED or high school diploma required.
• Valid and current Pennsylvania Driver’s License and Motor Vehicle Report (MVR) must show that Patient Concierge is a safe and responsible driver. Must also pass MVR check yearly. Must have knowledge of local streets and highways.
• Two (2) to five (5) years’ experience in providing high level customer service in a community-based outpatient setting, addressing, and resolving complex issues is required.
• Ability to communicate effectively in both oral and written communications.
• Polite and confident and patient demeanor.
• Ability in multi-tasking and time management
• Aptitude in resolving issues with a customer-focused orientation.
