POSITION SUMMARY

The "Housing Family Navigator" will work in conjunction with Pediatric providers and staff to support, provide information and offer ongoing housing assistance to families with children who receive pediatric care at BMC. Services include meeting mothers and caregivers to assess housing barriers (such as homelessness, rent arrears and/or housing accommodations), coordinate assistance with community partners, provide assistance and co-navigate cases with navigators, social workers and providers and track caseload status, among others. Responsible for outreach to families, meeting with families, assisting families in interactions with housing authorities and community partners, collecting and managing clinical and research data in a standardized format.

ESSENTIAL RESPONSIBILITIES / DUTIES:

Responsible for working with families:
  • Meets families to assess their housing barriers and needs
  • Follows up with families and community-based agencies via phone contact
  • Provides appropriate housing information, counsels and supports families
  • Provides and ensures successful referrals to community and governmental programs, including referrals to Metro Housing Boston, DHCD Emergency Assistance shelters, Family Aid, among others, etc.
  • Provides information materials to families related to housing programs
  • Demonstrates the knowledge and skills necessary to provide care to a culturally diverse patient population
  • Provides outreach to and collaborates with relevant community providers
  • Helps families interact with housing partners and complete forms.
  • Works with community-based organizations on behalf of families, helps families complete applications for community services.
  • Helps families to solve problems that interfere with obtaining care and support for their child and their family.

Responsible for working with navigators, social workers and providers:
  • Provides up to date information to navigators, social workers and providers regarding program and cases updates
  • Support co-workers in a co-navigation model to knowledge gaps and problem solving

Performs a variety of follow-up duties such as:
  • Maintains notes and records.
  • Maintains a log of children and families and documentation of contacts.
  • Maintains positive relationships with health care providers and community partners.
  • Assists team in making referrals and supporting family with follow through.
  • Serves as the central point of contact for formalized partnerships between BMC and local housing partners including Metro Housing Boston and Family Aid Boston.

Performs administrative tasks to include:
  • Recordkeeping, including filing encounter notes.
  • Collects and manages clinical data in standardized format.
  • Maintains a data management system ensuring timely and accurate data collection. Compiles quantitative and qualitative data daily, weekly, monthly and uses data to determine status of program activities.

Works cooperatively with other team members. Provides and receives constructive feedback from team members. Contributes to the development of new ideas that impact the program. Maintains patient confidentiality.

Meets hospital-wide standards in the following areas:
  • Conforms to hospital standards of performance and conduct, including those pertaining to patient rights, so that the best possible customer service and patient care may be provided.
  • Utilizes hospital's values as the basis for decision-making and to facilitate the division's hospital mission.
  • Follows established hospital infection control and safety procedures
  • Performs other duties as assigned or as necessary.

OTHER DUTIES:
  • Performs office related duties such as picking up mail, categorizing and delivering mail, storing and distributing office supplies, handling faxes, scanning, filing, photocopying collating materials, maintaining the update of policy manuals, etc.
  • Conforms to hospital standards of performance and conduct, including those pertaining to patient rights, to ensure that exceptional customer service and patient care may be provided.
  • Utilizes hospital's behavioral standards as the basis for decision making and to support the hospital's mission and goals.
  • Follow established hospital infection control and safety procedures.
  • Perform other duties as needed.

JOB DESCRIPTION CHARACTERISTICS

EDUCATION:
  • Bachelor's Degree (or equivalent combination of formal education and experience) required.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
  • None

EXPERIENCE:
  • Bachelor's Degree preferred. (*An equivalent combination of education and experience, which provides proficiency in the areas of responsibility, may be substituted for the college degree).

KNOWLEDGE AND SKILLS:
  • Bilingual/bicultural in Haitian Creole or Spanish preferred
  • Knowledge of housing community resources and agencies preferred
  • Excellent interpersonal skills; able to work on an interdisciplinary team
  • Cultural sensitivity and comfort with a wide range of racial and ethnic populations
  • An organized work style and ability to keep written documentation; proficient with standard Microsoft programs (i.e. MS Word, Excel, PowerPoint, Outlook) Epic and web browsers.
  • Ability to work in a multi-cultural, diverse, decentralized environment, as a member of a health care team and comfortable working with multi-cultural, low-income families
  • Strong communication (oral and written), interpersonal, organizational, and record keeping skills.
  • Ability to handle multiple tasks and responsibilities at the same time effectively.
  • Ability to interpret and record data and understand basic medical terminology.
  • Ability to be self-directed, prioritize essential tasks and balance numerous responsibilities.
  • Must have excellent interpersonal skills, including and ability to work with individuals from a variety of disciplines, and on a variety of levels.
