Per Diem Inpatient Specialist ,Transfer Team
Job description
We are looking to hire a per diem Inpatient Specialist, Transfer Team .
Responsibilities for this position include completing non-clinical tasks related to supporting the Inpatient Case Management and Vital Care Programs. These include performing all necessary clerical functions from the time of admission through discharge of the inpatient stay and inclusive of required follow-up care. Primary functions include updating and maintaining the hospital daily grid and hospital database and a support to the Inpatient Case Manager and Vital Care Manager.
Essential Duties and Responsibilities include the following:
- Answer Inpatient Hospitalist line and exchange phone lines.
- Prepare VP/Director meeting agendas, taking notes from meetings and distributing them out to staff.
- Maintain VP/Director calendars.
- Prepares agendas and make arrangements for meetings.
- Orders and maintains office supplies.
- Assists with Case Manager pay-roll.
- Manage PTO calendars for multiple campuses.
- Train Case Managers and Coordinators to computers systems.
- Process death Certificates.
- Process pharmacy refills.
- Page physicians for family members.
- Direct calls to the appropriate physicians.
- Process all admissions and discharges for designated hospital daily into authorization system.
- Enter admission and discharges into designated Hospital Logs and spreadsheets daily.
- Process all ambulatory authorizations for designated hospital at the direction of the C.M. Nurse.
- Process home health and/or DME discharge authorizations for members as needed and fax to appropriate vendor.
- Data enter notes from daily Lakeside Hospitalists’ Meeting into designated databases.
- Daily review patient information on designated logs for accurate admit, discharge date, etc.
- Checks eligibility and benefits when required.
- Prepares appropriate denial letter at the direction of the C.M. Nurse.
- Maintains confidentiality of all patients and medical/clinical information per HIPAA guidelines.
- Performs other duties as directed by management.
- Arrange and distribute monthly on-call case manager/Medical Director Schedule.
- Organize and distribute weekly Glendale Campus/Federal SNF schedules.
- Assist VP of Clinical Services with various projects.
- Work with Desert Oasis in regards to Coumadin Clinic to resolve physician or staff concerns; generate authorizations if needed.
- Manage and track Hospice patient accounts for the Glendale Campus/Federal SNFs, working closely with Project Manager.
- Generate reports, spreadsheets, and various contact information for upper management.
- Provide administrative and technical support to Inpatient and Vital Care Teams.
- Process Patient Keeper for PSJMC and PHCMC daily.
Education and/or Experience:

1. High school graduate required; Associates degree or higher preferred.
2. Knowledge of Healthcare and Managed Care required.
3. Typing 60 word per minutes with accuracy.
4. Knowledge of computers, faxes, printers and all other equipment.
5. Proficient in MS Office programs (i.e., Word, Excel, Outlook, Access and Power Point).
6. Ability to deal with responsibility with confidential matters.
7. Ability to work in a multi-task, high stress environment.
8. Ability to prioritize.
Education and/or Experience
High school diploma or general education degree (GED); and one year related experience.
