The Peaks Care Center is seeking Full Time Personal Care Provider or QMAPs to join our team!

In your QMAP role, you will work under the supervision of our Health & Wellness Director. QMAPs are responsible for administering medications to residents in a professional and timely manner per physicians’ orders.

As a Personal Care Provider you will be assisted in receiving your QMAP certification. You will work under the supervision of our Health & Wellness Director, with similar roles of a QMAP.

Primary Job Responsibilities:

  • Administering medication and completion of daily shift reports.
  • The ability to manage assigned tasks within the scheduled shift.
  • Direct visitors to the appropriate person and office.
  • Provide residents with medication administration and treatment assistance and services according to the health care provider's orders.
  • Supervise residents who self-administer medication while following community and regulatory guidelines.
  • Report any changes in residents' physical condition and/or behaviors.
  • Provide assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living.
  • Document daily log of assistance.
  • Follow the schedule of resident's needs set out by supervisor.
  • Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices
  • Perform other duties as assigned.

Qualifications:

  • High school diploma or equivalent required.
  • One (1) years of experience preferred.
  • Current Med Tech or QMAP certification preferred. Company can assist in obtaining certification. Certification must remain current during employment.
  • CPR certification
  • First Aide Certification

***HOURLY WAGE RANGE: $15.73-$18.01 Shift Differentials Ranging from $1.00 - $2.75!!!***
