The current vacancy is a Full-Time, Regular Pharmacist position in the Pharmacy.

At the Pharmacy, we are seeking dedicated Staff Pharmacists to perform a full range of professional pharmaceutical duties at SMMC, including medication therapy monitoring for SMMC patients. The Pharmacy serves both inpatient and outpatient clients at the hospital, outpatient clinics, a skilled nursing facility, and the County Correctional Facilities. As a Staff Pharmacist you will rotate through all areas of the pharmacy.

Reporting to the Supervising Pharmacist, you will be responsible for directing, selecting, preparing and distributing drugs, solutions and materials for medical and scientific use; directing supportive personnel involved in these same activities; providing clinical judgments necessary to ensure proper utilization and administration of medications in cooperation with the medical and nursing staff; and providing patient counseling and medication education services. This includes active involvement in the Antibiotic Stewardship program, pharmacokinetic dosing, medication counseling upon discharge, and many other non-dispensing functions.

Examples Of Duties

Duties may include, but are not limited to, the following:


  • Direct and participate in the stocking, inspection, storage, preparation, dispensing, and administration of medications in the patient care area and functionally direct assigned personnel and operations of the pharmacy area to which assigned.
  • Prepare intravenous (IV) medications for all nursing stations and chemotherapeutic IV medications for oncology out-patient clinics.
  • Prepare reports on activities within the section as required.
  • Maintain records of controlled medications as required by federal and state laws.
  • Assure that strict aseptic techniques are followed in preparing sterile solutions and that records are properly completed.
  • Advise patients of drug use, giving information on the medication regarding proper use, storage, common side effects, and drug interactions.
  • Monitor the safety and efficacy of drug therapy through the maintenance of medication profiles in a computerized data base.
  • Participate in management of medical emergencies, adverse drug interactions or reactions, acute and chronic disease states.
  • Participate in the education of health-care practitioners.
  • Provide drug information to prescribers and other health care practitioners; assist prescribers in the proper selection and adjustment of drug therapy.
  • Act as pharmacist-in-charge when so designated.
  • Participate in Pharmacy/hospital committees as required.
  • Initiate and participate in drug use reviews, quality assurance programs, patient care audits, clinical drug investigations, and development of systems for improved delivery of services.
  • Develop and utilize systems for cost effective fiscal management and reimbursement.
  • Maintain a perpetual inventory of all controlled substances and dispense them to nursing stations according to federal and state laws.
  • Estimate needs and order pharmaceuticals and chemicals.
  • Maintain and keep in order pharmaceutical and chemical supplies.
  • Perform related duties as assigned.

Qualifications

Knowledge of:


  • Prescription and non-prescription drugs and drug products and their indications, efficacy, dosage, mechanisms of action; fate and disposition (if known), major contraindications and potential side effects.
  • Influence of patient’s age, sex, concomitant disease states, concurrent drug therapy, foods and diagnostic procedures on drug activity and disposition.
  • Physiology, anatomy, pathology, chemistry and mathematics as they relate to pharmacy practice.
  • Major reference sources to answer drug information/ingestion questions.
  • Pharmaceutical manufacturing methods.
  • Legislation governing pharmacy practice and hospital pharmacy services/hospital operations as they relate to pharmacy services.
Skill/Ability to:
  • Compound and manufacture a wide variety of medications.
  • Prepare IV medications.
  • Prepare unit dose medications.
  • Learn and carry out pharmacy procedures and practices.
  • Identify, achieve, maintain and use those competencies directly relevant to his/her area of practice.
  • Establish and maintain cooperative working relationships and provide functional direction to para-professional personnel.
  • Effectively counsel patients regarding use, possible side effects and storage of medications.
  • Effectively advise and consult with medical, nursing and other personnel regarding properties of drug substances.
  • Use and evaluate information in appropriate reference sources and clinical literature.
  • Maintain records and prepare reports as required by federal and state laws.
  • Communicate clearly and effectively.
  • Understand and carry out oral and written instructions.
  • Deal tactfully and work cooperatively with hospital staff and patients.
Licensure/Certification:
  • License to practice pharmacy in California.

Open and Promotional. Anyone may apply. Current County of San Mateo and County of San Mateo Superior Court of California employees with at least six months (1040 hours) of continuous service in a classified regular, probationary, Confidential and Management extra-help/limited term, and SEIU or AFSCME represented extra help/limited term positions prior to the final filing date will receive five points added to their final passing score on this examination.

The examination process will consist of an application screening (pass/fail) based on the candidate's application and responses to the supplemental questions. Candidates who pass the application screening will be invited to a panel interview (weight: 100%). Depending on the number of applicants, an application appraisal of education and experience may be used in place of other examinations, or further evaluation of work experience may be conducted to group applicants by level of qualification. Applicants who meet the minimum qualifications are not guaranteed advancement through any subsequent phase of the examination. All examinations will be given in San Mateo County, California, and applicants must participate at their own expense.
