Dignity Health Home Health services provide comprehensive care for patients who have recently been discharged from the hospital or homebound individuals needing professional nursing and/or rehabilitative care. Responsibilities

The Home Health Physical Therapist will work together with a multi-disciplinary team to provide care, identify functional goals and outcomes, and increase independence for our patients and their families. Home physical therapists specialize in aiding patients who have limitations in function by examining and evaluating the patient to develop and execute a plan of care in collaboration with the physician.

Home health physical therapists will have the ability to work autonomously, and participate in care coordination with the treatment team, physicians, and referral sources. The physical therapist will work with patients, their families and the care team to achieve recovery and to re-engage the patient with their community.

Qualifications

Minimum Requirements:
  • Minimum of two (2) years cliniical experience within the last five (5) years
  • Evidence of ability to function with a high degree of independence and competence
  • Manages audits, feedback on evaluations, and percepts
  • Two (2) to nine (9) years of experience of homecare based
  • Graduate of an accredited physical therapy program
  • Current California license in Physical Therapy (LIC PT)
  • Current CPR through American Heart Assocation Certification (CT CPRBLS)
  • Current California Driver’s License (CA DL)
  • Clean driving record with proof of liability insurance for automobile
Special Skills:
  • Demonstrates good interpersonal skills with fellow staff members, physicians, hospital departments and the general public.
  • Demonstrates competent working knowledge of the standard practices, procedures and equipment utilized in the
    department.
  • Ability and willingness to actively participate in departmental and hospital programs.
  • Demonstrates self-motivation and ability to achieve results through good organizational skills and prioritization of work.
  • Ability to work independently and be a functional team member in a team oriented environment.
  • Ability to mentor/coach student interns or oversee their clinical experience.
  • Ability to be flexible with caseload within any given day, as needed for the benefit of the department.
  • Promotes P.T. services in the community as appropriate.

Pay Range
$44.72 - 64.85 /hour
