Summary
Population Health Data Analyst Manager is a pivotal position using a variety of data sources to blend and join data for risk stratification to improve the health outcomes and quantify cost savings. This position will provide critical analyses for clinical quality improvement programs that seek to identify challenges and best practices among our network of health centers. This role will provide technical expertise to health center staff for optimizing the use of available clinical and claims data. Assist with the development, implementation, and maintenance of accurate data stewardship from user inputs to reporting outputs, including the maintenance of databases used to house curated data sets and health plan data. Ability to extract data using a variety of commercially available tools including MS Excel, SQL Server Management Studio (SMSS), and can utilize data visualization tools such as Tableau, Power BI, etc. This position is required to report in-office M-F.
Responsibilities
- Develop and implement databased, data collection, data analysis and other strategies that optimize statistical efficiency quality and patient stratification
- Work with Population Health Collaborative and Clinical Quality Improvement (CQI) team to strengthen and improve access to care, operational excellence, and health outcomes
- Responsible for the preparation and accurate and timely delivery of reports and ad hoc data requests/extracts to various internal and external stakeholders.
- Create and maintain documentation relating to master data management including report specifications and data dictionaries. Documents the methodologies used to develop reporting outputs, including demonstrating the flow of data to supervisors for review purposes.
- Collect, analyze and report on data to meet business operation’s needs. Identify new source of data and method to improve data collection, data profiling and reporting. Acquiring data from primary and secondary sources and maintain data systems.
- Performing analysis to assess quality and meaning of data. Prepare statistical report/dashboard for the management stating trends, patterns, and predictions using relevant data
- Work with HFF Project Manager and Clinify Representatives to analyze data, perform queries/extracts and validates quantitative data regarding health center services, operations, and payment; identifies opportunities to evaluate programs for effectiveness and quality.
- Prepare final analysis reports for the stakeholders to understand the data-analysis steps, enabling them to take important decisions based on various facts and trends.
- Provide monthly UDS data reports to management team.
- Participate in the monthly PHC and CQI meetings to discuss HEDIS, evaluating performing measures outcomes, identifying opportunities to close gaps and analyzing patient engagement metrics.
- Design simple to moderately complex, flexible data models (conceptual and logical).
- Collaborate with the clinical team to evaluate risk stratification data.
- Provide quality and cost saving data analysis reports to the leadership team.
- Create/Capture documentation (metadata) that is up to date.
- Provide reporting and analytical services to executive leadership, providers, management and staff.
- Participate in Plan, Do, Study, Act (PDSA) projects as assigned.
- Work in conjunction with the leadership team to develop workflows that strengthen clinical outcomes.
- Lead author with completing the annual UDS statistical report and submission to federal grantor
- Work closely with Clinify customizing population health platform for SDOH for housing and primary care.
- Support departmental activities related to analyzing operational and financial data.
- Maintains disease registries and analyzes panels at regular intervals to facilitate proactive interventions with clients who meet clinical or quality criteria
- Collaborate with others to develop and maintain reporting and tools for measuring performance; performs basic and complex analyses to monitor health care quality, day-to-day operations; supports compliance with regulatory reporting.
- Reviews data to determine operational impacts and needed actions; elevates issues based on trends and/or areas for improvement needing increased oversight.
- Analyzes data to assist in delivering optimal healthcare management and decision making.
- Designs, produces and interprets dashboards, charts and other presentation formats used for decision making and to track and improve data quality.
- Maintain awareness of and familiarity with the organization’s services, information, documentation and reporting issues and requirements.
- Oversee the tracking of HMIS, Provider, Ryan White Care ware and other statistical databases.
- Supervise the Data Entry Specialist with data sourcing and queries.
- Collaborates with leadership to define and standardize clinical performance metrics across the system.
- Conducts statistical inference analysis and testing to develop & evaluate data driven hypotheses and results, including analysis for leadership team to identify critical issues/questions around clinical improvements related to value-based imperatives.
- Analyzes data using descriptive and inferential statistics to create analyses, dashboards, scorecards, graphs, tables, and reports that define opportunities for improvement and make appropriate recommendations.
Minimal Qualifications
- 3+ years in healthcare reporting and analytics
- Bachelor's Degree in Computer Science, Finance or Information Systems.
- Excellent data management and analytic skills
- Well-developed programming and statistical skills, including the ability to prepare accurate reports using data query, analysis, and presentation tools, such as SQL; MS SQL; Oracle MS Oracle, R, MATLAB, Python, Excel; MS Access; R; SPSS; and Tableau.
- Knowledge of clinical quality measure specifications and programs such as NCQA PCMH; HEDIS; UDS • Experience with population health platforms
- Working knowledge of programming languages such as SQL, JavaScript, HTML, etc. is desired
- Preferred experience with Meaningful Use, Quality Data, CMS reporting (Medicare/Medicaid) and MIPS
- Preferred experience with claims data analysis
- Strong mathematical skills to help collect, measure, organize and analyze data
- Technical proficiency regarding database design development, data models, techniques for data mining, and segmentation.
- Experience in handling reporting packages like Business Objects, programming (JavaScript, XML, or ETL frameworks), databases
- Proficiency in statistics and statistical packages like Excel, SPSS, SAS to be used for data set analyzing
- Knowledge of how to create and apply the most accurate algorithms to datasets in order to find solutions
