1+ years as medical clinic admin/receptionist or 1+ years’ experience in a health plan call/contact center.

1 year experience in answering a multi-line

Valid Driver’s License- Clean/ Safe Diving record. Must have reliable personal transportation and current insurance coverage.

Employee will support Population Health service delivery and continuity of care through schedule maintenance, patient outreach/enrollment, monitoring fax boxes and routing to appropriate staff member, mailing patient communications, and general administrative support. Leads outreach/enrollment initiatives and supports departmental goals through customer service and administrative support functions. Population Health Liaison will have a patient-oriented mindset and will act as an ongoing liaison between patient, multispecialty care centers and approved facilities. Population health liaison must be comfortable working in a call center/desk setting, skilled with fielding inbound and outbound patient calls, able to work with a cross functional team, and able to prioritize daily tasks.
